PHMC is Hiring a Case Management Supervisor Near Philadelphia, PA

PHMC is proud to be a leader in public health. PHMC requires that all employees are fully vaccinated with the Covid-19 vaccine before the first day of employment. 

We are seeking a full-time Case Manager Supervisor to provide supervision to emergency housing case management staff. The Case Manager Supervisor will be responsible for supervising case management staff at 2 emergency housing programs: Darlene Morris (Love and Care) and ACTS Family Shelter. Case Management Supervisor will ensure that all Case Management staff adheres to all outlined responsibilities included in the Office of Homeless Services Case Management Standards along with practices/services aligned with the National Association of Social Work’s Code of Ethics. Reports to Director of Case Management and Rapid Re-Housing Services. 

Responsibilities:

  • Provide all supervisory duties for case management staff at the Darlene Morris, ACTS Single Facility, ACTS Family Shelter, and PHMC-12 including scheduling and monitoring of work duties, clinical and administrative supervision, and performance management.
  • Carry a caseload of no more than 15 clients.
  • Ensures that all housing assessments for program participants are submitted within the outlined timeframe.
  • Updates and maintains quarterly reports (i.e., housing spreadsheet, HAP report, and performance measurements) to be provided to the Director of Case Management and Rapid Re-Housing Services.
  • Identify compliance and production problems, and consult with Dir. of CM and RRH Services for corrective action plans.
  • Act as an arbitrator when problems occur between participants and Case Management staff.
  • Complete all Healthy Baby Program surveys for all babies between the ages of 0 and 4 months.
  • Monitor all early intervention referrals made by provider agency and maintain collaborative contact between program participants and early intervention staff.
  • Participate in interagency meetings (i.e., DHS, FaSSt etc.)
  • Act as a liaison for existing and/or new OHS initiatives including the Healthy Baby Program and Philadelphia Infant Toddler Early Intervention Program to ensure client and program compliance.
  • Interpret and explain rules and regulations to staff, and provide assistance (where necessary) with managing case management workloads.
  • Respond to both informal and formal grievances from program participants, outside agencies, emergency housing staff, and OHS staff and relays all pertinent information to Director of CM and RRH Services.
  • Review and assess all daily client related incident reports for necessary weekly Client Progress Review Meetings.
  • Conduct weekly Client Progress Review Meetings and ensure that all outcomes are documented within the HMIS system.
  • Participates in monthly supervision with the Manager of Shelter Case Management.
  • Conduct quarterly client record audits of case managers to ensure compliance with OHS Case Management Standards.
  • Collaborate with community based and/or social service agencies, as appropriate.
  • Provide monthly documented supervision to Case Management staff.
  • Adheres to OHS CM standards with regards to completing Protocols and Appeals if applicable for non-compliant participants.
  • Assist Case Mangers with developing appropriate interventions to assist participants with compliance with Emergency Housing and OHS Case Management Standards.
  • Using Sanctuary Model, the Case Manager Supervisor will facilitate monthly community meetings with the Shelter Director for program participants.

  • Familiarity with Client-Centered, Trauma Focused, and Housing First Models

Education Requirement:

  • Master’s degree in Social Work, MSW or MSS required

Salary:

  • Commensurate with education and experience 
  • Grade 19
