COMMUNITY BEHAVIORAL HEALTH is Hiring a Clinical Care Management Supervisor - Integrated Care for Kids Near Philadelphia, PA

The Clinical Care Management Supervisor is responsible for the daily supervision and oversight of clinical team members. The Clinical Care Management Supervisor will ensure that the assigned Care Manager(s) are working to enhance physical and behavioral health care coordination and assist with mitigating social determinants of health (SDOH), as well as ensuring that services are authorized according to medical necessity criteria and in accordance with the CBH mission and contractual obligations.

This position is a hybrid position and will be required to spend some time on site at St. Christopher's Hospital (160 E Erie Ave, Philadelphia, PA 19134).

Essential Functions

  • Build and maintain a strong functional clinical team through effective recruiting, training, performance management, coaching, team building and succession planning.
  • Manage day to day operations of Clinical Care Managers and other direct reports. 
  • Monitor quality of assigned work reviews and provide feedback.
  • Collaborate across clinical teams to facilitate and coordinate care.
  • Collaborate across functional areas of CBH to operationalize changes in policy and procedure.
  • Develops specific protocols for communication between Clinical Care Managers.
  • Facilitate communication with internal and external stakeholders.
  • Responsible for oversight for on-site community-based care management activities.
  • Uses data driven decision-making and works to ensure accuracy of member level data and timely communication of data.
  • Collaborates with other Clinical Supervisors to develop consistent expectations and supervision to Clinical Staff.
  • Obtains and reviews data relevant to members served by the team, clinical operations and staff monitoring to guide interventions.
  • Models and requires exceptional customer service in work with members and external stakeholders and/or interdisciplinary team.
  • Documents supervision notes and next steps consistent with standards in the field. 
  • Perform on-call duties as instructed.
  • Completes other duties as assigned. 

Team Specific Responsibilities:

  • Prepares and trains Clinical Care Managers to complete social determinants of health screening for members..
  • Coaches direct reports on member engagement techniques.
  • Ensure that Clinical Care Managers are reviewing cases and identifying appropriate immediate and longitudinal needs of members.
  • Oversees implementation of population stratification approaches.
  • Works collaboratively with physical health entities and system partners to ensure smooth referral and communication process. 

Position Requirements: 

  • Education: Master’s degree in Social Work or related field
  • License/Certification: License eligible and willing/able to obtain licensure within the first year of employment; PA Child Abuse Clearance required
  • Preferred Licensure: Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW)
  • Relevant Work Experience: 3 years post graduate experience with a minimum of 6 months of supervisory experience.
  • Skills:
  • Ability to positively lead a team
  • Community Based/Clinic Setting experience. 
  • Strong communication skills
  • Strong organizational skills
  • Ability to multi-task in fast-paced environment
  • Proficient in MS Office and able to type at least 20 words per minute
  • Compliant with HIPAA regulations

Work Conditions (including travel, overtime required, physical requirements and occupational exposure): 

  • Requires ability to work shift schedule including days, nights and overnights; schedule may vary.
  • Requires ability work off-site as needed.
  • Requires ability to travel, including out-of-state and overnight travel, as needed.
  • Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
  • Ability to utilize computers and office equipment to complete daily work responsibilities.
  • Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
  • Must be able to adapt to continuous changes/demands of the job.
