Position Summary: Community Treatment Teams (CTT) is mandated to provide service to those individuals within the behavioral health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric and legal conditions; extensive use of services and lack of follow through with treatment. The Blended Enhanced (BE) Team works with individuals who measure within the mid to at the high level of medical necessity as defined by the state Adult Environmental Matrix.

The Case Management Specialist (CM Specialist) provides case management supports based on the Community Support Program of Philadelphia with assisting individuals’ in developing skills required to enhance his/her comfort and abilities to function as a productive member of the community. The CM Specialist takes the lead in one of five (5) areas of specialty: vocational, housing, D&A, forensic or Benefits/Entitlements; and works in collaboration with the primary case manager to identify goals, develop, implement and monitor the service plan. The CM Specialist will ensure that the mission, goals and philosophy of the CTT are operationalized within the team. 

The incumbent works with individuals in their community environment assisting them in understanding, acquiring and maintaining independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs; (e) obtainment of benefit entitlements and the skills to manage same and (f) vocational and educational needs.

 Duties and Responsibilities:

Assesses participant’s strengths, needs and wants, utilizing instruments to operationalize data on the participant’s behalf, involving all treatment team principals for comprehensiveness.

· Monitors and update individual’s monthly progress toward attainment of identified goals according to regulatory standards and all related activities documented on appropriate agency forms

· Completion of required documentation in timely manner. This includes completion of Strengths Assessment and client-centered Recovery Management Plan for each assigned individual and 24 hour turnaround on all service documentation

· Periodically completes Environmental Matrix to determine level of care needed and delivers services according to individual’s needs; ensuring appropriate frequency of contact

· Works collaboratively with multidisciplinary team internally on daily basis and external, as needed, to ensure coordination between systems 

· Help participants make informed decisions by acting as their advocate regarding their clinical status and treatment options

· Liaises, communicates with and represents participant via telephone contacts and face-to-face meetings

· Participates in in-patient treatment team and discharge planning meetings; monitors individual’s treatment and progress during hospitalizations

· Provides support and resources for participants; good working relationship with support networks, government resources and community resources

· Accompanies participants to appointments (financial entitlements, medical, dental, court, probation, etc.) to provide support and assistance

· Acts as team specialist in one of the following areas: housing, benefits, D&A or vocational; including attending periodic specialist meetings as required

· Participates in daily team meetings and Clinical Care Meetings to problem solve around persons needing extensive services for specific times 

· Attends in-service and other trainings in order to meet mandated training hours

· This includes Open Forum and other required agency meetings, DBH or Community sponsored training

· Participates in F.L.I.P. (Field Led Integrated Project) in the delivery of case management services to participants

· Provides after hours work including: a) participation in weekly on call rotation schedule to permit 24 hour/7 day a week access to service; b) and participation in evening and weekend shift work schedules required for delivery of services to CTT’s assigned caseload

· Adheres to the policies in the use of computer technology and all tele-communication devices

Skills Required: 

Must have good verbal/written communication skills and work well with others; good, creative problem solving skills; ability to work independently and be flexible/adaptive in handling changing priorities in a fast paced work environment; computer skills preferred.

Education and Experience: 

Bachelor’s degree in (including but not limited to) sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling or education with two (2) years’ related experience preferred. Expertise in behavioral health with one (1) or more of the following required: criminal justice system, substance abuse, vocational, geriatric, MR, housing or entitlement issues.
