Essential Duties:

• Provide patients with personal care including bathing, grooming, dressing, and hygiene.
• Assist with daily living activities such as meal planning and preparation, household chores, and running errands.
• Monitor patient vitals and report any changes in health status to the nursing staff or RN.
• Assist with prescribed exercises and administer medication under the supervision of a nurse.
• Provide emotional support to patients and their families.
• Maintain patient records, documenting each visit accurately and confidentially.
