FT Social Worker (MSW)!!!!!

Schedule: M-F

Location: 330 S. 9th Street – Penn Neurology PAH

Department: Neurology

Summary:

  • We are recruiting for a social worker for our Penn Comprehensive ALS Center in the Department of Neurology at PAH. The center is designed to provide research-based, comprehensive care for ALS patients and their families throughout the course of their diagnosis. The team consists of experienced providers in multiple disciplines including: neurology, pulmonary, physical and occupational therapy, nursing and genetic counseling. People with ALS who come to Penn for treatment benefit from personalized care throughout the course of the disease. The social worker is responsible for collaborating and coordinating psychosocial support through the complex diagnosis. The individual is responsible for understanding and assisting patients and families navigate biopsychosocial needs related to their diagnosis and through they ALS journey. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers (NASW).

Responsibilities:

  • Coordinates and collaborates with the various members of the multi-disciplinary team following patient’s initial visit ensuring needs are met following initial diagnosis. Ensures patients and family members have access to all equipment, education and resources that are needed.
  • Collaborates with community resources, behavioral health team and various outside organizations to support patient and family through their journey.
  • Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting.
    • Collaborates and cooperates with other members of the interdisciplinary health care team to ensure continuity of care.
    • Identifies and attends to the psychological needs of patients and families.
    • Provides counseling to patients, families and groups.
    • Responsible for documentation in the medical record, statistics and reports as required.
    • Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
    • Maintains relationships with community agencies and keeps up-to-date on resources.
    • Uses effective interpersonal skills in all interactions
    • Enhances own professional development and maintains formal and informal continuing education for self improvement.
    • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
    • Other duties as assigned to support the unit, department, entity, and health system organization

    Credentials:

    • Licensed Social Worker preferred

    Education or Equivalent Experience:

    • 0-1 years Health Care experience in field instruction and/or employment in a health care setting. Required
