Duties:
To provide nursing carein accordance with the patient's plan of care to include comprehensive healthand psychosocial evaluation, monitoring of the patient’s condition, healthpromotion and prevention coordination of services, teaching and trainingactivities and direct nursing care.
Job Conditions:
Must have a driver’slicense and be willing and able to drive to patient’s residences.
The ability to accesspatients’ homes which may not be routinely wheelchair accessible is required.Hearing, eyesight and physical dexterity must be sufficient to perform aphysical assessment of the patient's condition and to perform and demonstratepatient care.
Physical activities willinclude, walking, sitting, stooping, and standing and minimal to maximumlifting of patients and the turning of patients.
The ability tocommunicate both verbally and in writing in English is required as frequentcommunication by telephone and in writing is involved.
Qualifications:
Must be a graduate froman accredited School of Nursing.
Must be licensed inPennsylvania as a Registered Nurse.
One or more years ofexperience in community/home health agency or in a hospital setting ispreferred.
Must have knowledge ofMedicare and Medicaid guidelines.
Must have a workingknowledge of home health care and the principles and techniques of professionalnursing and required documentation that pertains to it.
Should be skillful inorganization and in the principles of time management and have knowledge ofmanagement processes.
Must be able tocontribute to the quality of care being rendered through constructivecommunication with nursing managers and staff.
