Pearl Health Center is an integrated health clinic located in the Hillside neighborhood. Pearl Health Center is seeking a Community Health Worker to join our team and expand our Community Health department. At Pearl Health Center our Community Health Workers work alongside Primary Care Providers, Acupuncturist, Chiropractors, Behavioral Health Specialist, and Mental Health Providers to create the most comprehensive and integrated health care possible.
The Community Health Worker would be responsible for
· Conducting outreach and engage with patients to promote health education and awareness
· effectively communicate health information to diverse patient population
· Understand social factors that impact health
· conducting interviews to determine social determinants of health
· provide referrals to community resources
· be a liaison between the community resources and the patients
· tracking referrals
· documenting encounters with patients within the EHR.
· effectively communicating with healthcare professionals
This position can either be full-time or part-time.
Previous Community Health Care experience preferred but not required. If you are a dedicated individual who is passionate about improving community health then this is a great opportunity for you to join our team as a Community Health Worker and make a positive impact on the lives of those you serve. Traditional Health Worker registration is required within the first year.
