Parenting Resource Center, Inc. is Hiring a Case Manager Near Austin, MN

POSITION SUMMARY:

This position supports the work of the Parenting Resource Center, Inc. (PRC), a leading nonprofit committed to strengthening community through resources, services, and prevention education to promote healthy, safe lifestyles for families within communities.

This Case Manager is a grant funded position which will provide intensive case management to qualifying Refugee FOCUS participants who reside in the Southeastern Minnesota region as part of Welcome Center’s Advocacy and Social Services program. The Welcome Center is a multicultural center that welcomes newcomers, supports them in their transition into the community, and provides access and opportunities

ESSENTIAL FUNCTIONS:

1. Provides case management and employment services to program participants in coordination with MFIB service providers.

2. Practices shared decision making with families they will be working with.

3. Demonstrates (through words and actions) belief that all individuals can be successful and be willing to work to remove barriers for children and families to ensure success for all participants.

4. Understands any stressors, including the psychosocial stressors that refugees and other humanitarian protected status families may experience.

5. Understands, or shares, diverse cultural values such as trust, familism, and personalism.

6. Facilitates client intake and onboarding.

7. Creates Individualized Goal Plans for program participants through collaboration with MFIB partners.

8. Provides regular ongoing, remote, or in-person connections with program participants and regular ongoing checks on measurable progress markers identified in the participants’ goal plans.

9. Utilizes the Check and Connect service model and documentation tool.

10. Maintains regular consultation and communication with MFIB ES/FSS providers per participant needs.

11. Maintains regular coordination with resettlement network service agencies, per participant needs.

12. Maintains a caseload of 20-25 families at any given time.

13.Advocates for children and families by offering appropriate feedback, referrals, and regularly checking in with the families to answer questions, share information about the importance of education, share economic opportunities and access to opportunities.

14. Maintains completed documentation and update case files.

15. Develops systems and programs, which encourage participant and partner feedback for improving ways to serve participant. Creates trauma informed environment within the program. Provides a safe environment for all participants.

16. Ensures proper implementation of Refugee FOCUS program in accordance with grant work plan.

17. Provides written strategic plan to meet program goals. Develops and monitors program budget to meet fiscal objectives.

18.Compiles program statistics. Monitors and evaluates the effectiveness of participation in the program.

19.Assists in the marketing and distribution of program information, may organize and schedule program registrations.

20.Attends community meetings, gaining understanding of community resources and networking with supports in SE MN. Develops and maintains collaborative relationships with communities and service organizations and represents the Parenting Resource Center through community participation.

21. Models relationship-building skills in all interactions. Responds to all participant, community and partner inquiries and complaints in a timely manner.

22.Communicates the mission, vision, and values of the Parenting Resource Center to everyone who encounters the program and organization.

23. Performs other duties as assigned.

Develops plans and manages best practices through engagement of partners and participants. Effectively creates and manages budgets. Be accountable for high-quality results using a formal process to measure progress.

Personal Growth: Shares new insights. Facilitates change; model’s adaptability and an awareness of the impact of change. Utilizes non-threatening methods to address sensitive issues and inappropriate behavior or performance. Has the functional and technical knowledge and skills required to perform well; uses best practices and demonstrates up-to-date knowledge and skills in technology.

QUALIFICATIONS:

Minimum Qualifications:

1. Two year degree in a human services field and one year experience in program development, implementations, case management and/or experience working with children and families.

2. Possess strong, effective interpersonal and communication skills.

3. Proficiency in Microsoft Office, email, and electronic calendar; proficiency in basic computer use.

4. Ability to handle and keep confidential information.

5. Ability to work independently with minimal supervision.

6. Solid organizational and time management skills.

7. Ability to travel within a 45 mile radius of Austin for meetings with clients and partners.

Preferred Qualifications:

1. Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service degree.

2. Two years of experience providing case management services.

3. One year of experience working with children and families.

4. Preferred but not required current LSW (Licensed Social Worker) or working towards completion of LSW.

Typical requirements within 60 days of hire include: completion of: CPR; First Aid; and AED certifications.

6. Fluency in language in addition to English is considered an asset.

7. Ability to relate effectively to diverse groups of people from all social and economic segments of
the community.

8. Ability to travel within a 45 mile radius of Austin for meetings with clients and partners.

LICENSE/CERTIFICATION REQUIRED:

CPR, First Aid and AED certification upon hire or within 60 days of hire

Must pass a state criminal background check with no pending criminal allegations against them.

Must have a valid driver's license, reliable transportation and motor vehicle insurance.

Must have a clean driving record.

· While performing the duties of this job, the employee is often required to: climb stairs, bend, stoop, push, squat, sit, crouch, walk, kneel, twist, pull, reach with hands, sit, stand for an extended period of time, run, throw, swim, Climb ladders, walk, shovel snow, plow snow, have finger dexterity, grasp, talk, hear, have visual acuity, vocal projection and perform repetitive motions..

· While performing the duties of this job the employee is exposed to weather conditions prevalent at the time.

· Must be alert at all times, keeping safety in mind. Must possess acceptable hearing and visual capabilities in order to monitor environment and well-being of members.

· Must be capable of working under pressure in a somewhat disruptive environment.

· May be required to sit, stand, or maintain physical activity for extended periods of time.

· Sufficient strength, agility and mobility to perform essential functions and to supervise program activities in a wide variety of indoor and outdoor locations (depending upon the programs).

· Ability to travel within a 45 mile radius of Austin for meetings with clients and partners.
