About Our Job
This is a limited position with an end date of 12/31/2024 and the potential to be extended.
The Denver Sheriff Department is the largest Sheriff Department agency in the state of Colorado and is comprised of many divisions and diverse operations with over 1000 uniformed and non-uniformed staff members. These operations include two separate jails, security for the District and County court systems, state inmate transportation, extradition duties, fugitive and K-9 units and security at Denver Health Medical Center.
The Denver Sheriff Department is seeking a Case Manager. The Case Manager will assess, counsel, monitor, evaluate, and provide case management to clients within the Denver jail facilities. The Case Manager will meet with inmates individually to address reentry planning, screen and assess risk and needs, navigate resources, write transition plans, and conduct group education program services to target interventions based on assessed criminogenic needs. This role will work collaboratively with clients, Deputy Sheriff's, the Programs Team as well as community partners to develop unique case plans.
This position will work at our County Jail at 10500 East Smith Road, Denver, CO 80239 and occasionally assist with clients at our Downtown Detention Center located at 490 W Colfax Ave. Denver, CO 80204.
Successful candidates must complete and pass a background investigation that encompasses the applicant's employment and criminal history.
Job responsibilities will include:
  • Developing individual case plans with assigned clients, which may include participation in support groups and/or cognitive behavioral/educational classes, case management services, and/or referring client to community agencies that meet individual needs.
  • Communicating and coordinating reentry services with resource providers in the community.
  • Preparing and conducting various cognitive/behavioral classes, facilitating groups, and selecting the curricula for individual groups and classes.
  • Maintaining client files and records consistent with departmental policies and local, state, and federal rules and regulations.
  • Provides individual and group education to clients in order to assess a client's motivation for programming and change, assist clients in the change process, and optimize client outcomes, discusses alternative methods of coping with stress, anxiety, and other issues, and utilizes support systems to assist in the reentry process including family members and/or friends.
  • Observing and documenting client behavior and physical conditions, and reporting abnormalities and/or problems to sworn and/or medical staff reports any abnormalities and/or problems to medical staff.
  • Implementing behavior management programs/techniques and appropriate intervention techniques
  • Initiating reentry planning based on the client's identified needs such as housing, employment, continuation of substance misuse and behavioral health counseling, and other services, and referring clients to appropriate programs and/or services.
  • Developing and maintaining relationships with community providers to enhance successful warm handoffs (referrals) and post-release engagement.
  • Performing other related duties as assigned.
About You
Our ideal candidate will have:
  • 1-3 years of experience working with milieu management, substance misuse education, and cognitive behavioral interventions
  • Experience working with the justice-involved population
  • Experience working in a jail or prison setting
  • Prior class facilitation experience
  • Microsoft Excel and Word Skills
Below are the minimum requirements used to determine eligibility for this position:
  • Education Requirement: Bachelor's Degree in Psychology, Sociology, Human Services, Criminal Justice, or a related field
  • Experience Requirement: One (1) year of experience in performing case management including completing comprehensive needs assessments and developing and implementing case plans. Or one (1) year of experience in case work and counseling in areas such as Diversion, Probation, alcohol and drug abuse, community-based treatment case management, or a closely related area.
  • Education & Experience Requirement: One (1) year of the appropriate type and level of experience may be substituted for each required year of post high school education. Additional appropriate education may be substituted for the minimum experience requirements.
  • Licensure & Certification: By position, requires eligibility to possess a Certified Addictions Counselor (CAC) certificate or an Alcohol and Drug Evaluation Specialist (ADES) Certification from the State of Colorado at the time of application. Licenses and certifications must be kept current as a condition of employment.
