Working together withmanagers and other professional staff, the Case Manager will oversee compliance,coordination, and supervision according to regulatory and policy requirements toensure continuity of patient care and business operations.

Primary Responsibilities will Include:

  • Assess clients' physical and mental wellness, personal needsand preferences, and develop plans for improvement
  • Work with clients, family and friend support networks, andhealth care professionals to put care plans in place
  • Listen to clients' concerns and provide counseling orintervention as required
  • Record clients' progress, chart referrals and schedule homevisits
  • Evaluate clients' progress periodically and make adjustmentsas needed
  • Communicate withclients/caregivers to schedule supervisory visits every 90 days.
  • Communicate with clients andcaregivers regarding their care plan, changes/updates.
  • Prepare and present reports of case management activities tosuperiors or relevant authorities
  • Manage ongoing relationships withclients, client families and field staff.
  • Act as an alternate for clientcare services under emergency circumstances, such as call offs immediate startneeds.
  • Follow up with former clients for feedback on services andensure they are in good physical and mental health
  • Interviewing potential fieldstaff employees
  • Conduct on-boarding, andorienting of new caregivers
  • Oversee the caregiver mentorprogram.
  • This role is a combination fieldand office position.
  • Must participate in the On-Callrotation.
  • Lead scheduling functions toserve all of our clients and caregivers.
  • Work closely with HR andMarketing to establish and satisfy staffing needs.
  • Attend educational programs, conferences and workshops toimprove on existing job knowledge

Requirements

  • Minimum of one (1) year of HomeCare experience
  • Knowledge regarding Medicaid and Home Care state regulations
  • Availability to cover shifts and assist with admissions whenneeded
  • Strong interpersonal andcommunication skills and the ability to work effectively with the team,patients and their families
  • Excellent organizational andtime management skills
  • Problem-solving skills andability to multi-task
  • Proficiency in Microsoft Officeprograms (Excel, Word, Outlok, etc.) and general computer knowledge
  • Valid driver’s license andreliable transportation to make visits to patients’ homes

Work Schedule: Mondaythrough Friday, 8:00 am-5:00 pm. Hours may vary depending on Client and/orbusiness needs.
