PA CARE AT HOME SERVICES is Hiring a Patient Care Coordinator Near Philadelphia, PA

  • Job Summary: The role of a Patient Care Coordinator is to ensure that efficient communication is being made between the patient/consumer and PA Care at Home Services. A Patient Care Coordinator is the first contact that the patient/consumer has when they are our patient/consumer. They have an assigned caseload they are always to be courteous and helpful.The PCC reports directly to the their immediate Supervisor/HR/CEO/ECEO.

Responsibilities:

  • Maintains a caseload of up to 100 patients.
  • Responsible to respond to referral inquiries promptly and complete the full Intake cycle process for services, non-skilled services and private pay and case manage.
  • Receives incoming calls and makes outbound calls to follow up lead referrals, state agencies, physicians, and partners, etc.
  • Responsible to understand and comply to various state waiver programs and private pay initiatives.
  • Develops professional relationships with outside agencies to better assist incoming consumers.
  • Work with Supervisor/CEOs/marketing to effectively funnel all lead referrals through the intake department.
  • Identify opportunities to be proactive in consumer cases and build relationships.
  • Communicate customer service feedback to Supervisor/CEO/Partners.
  • Maintaining a positive and empathetic and professional attitude towards employees and consumers at all times.
  • Monitors gaps and absence of activity on HHA report coming from Coordinators (PCA or Consumer) to ensure that services are rendered (Missed Visits and Units).
  • Work in collaboration with the PCA Coordinators, and Consumer/Patient Coordinators to ensure that the proper follow up was completed before requesting discharge.
  • Maintains communication to ensure that all incoming PCAs are in compliance and that documentation is acquired by communicating with PCA Coordinators.
  • Communicates with consumers daily and family members or as needed.
  • Follow up/Documentation on Hospitalization or medical changes
  • Creates and manages Plan of Cares.
  • Assist with grievances/appeals/complaints.
  • Assist with authorizations/calendar and schedule changes.
  • Demonstrates commitment to excellent internal and external customer service.
  • Adhere to policies, procedures, and compliance requirements of PA Care at Home Services for the Department of Health, OLTL and MCOs.
  • Check eligibility and waiver.
  • Reporting to insurances and internally.
  • Other duties as assigned

Job Type: Full-time

Pay: From $15.00 per hour

Expected hours: 40 per week
