Outreach Health is Hiring a Care Manager Near Edinburg, TX

Job Description

As part of the care team, the Care Manager is responsible for the delivery of services to all clients on his/her caseload by supervising caregiver activities. You are responsible for the training, development, and supervision of caregivers to ensure clients receive the highest level of care. You will be responsible for managing incidents, complaints, injuries, and act as back-up supervisor as needed. Supports company initiatives by adopting new approaches, practices, and processes to deliver standards of excellence and operational quality on a consistent basis. Essential Functions

· Maintain quality service by following Outreach Standard Operating Procedures to onboard new clients timely and ensure accuracy. This includes duties such as: schedule initial visits with clients and caregivers, developing a plan of care, and conduct caregiver orientation and training

· Conducts client visits in accordance with program requirements for supervision and evaluation of services to ensure client safety in the home

· Prepare caregiver work schedules on a regular basis and maintain necessary documentation

· Assists in monitoring adherence to the prepared caregiver work schedule

· Manage and maintain a client caseload, via the Electronic Health Record (EHR) to track and report scheduling, timesheets, EVV compliance and supervisory visits

· Identify and address client concerns timely and with empathy

· Effectively supervises the caregivers in all aspects of performance of their jobs, including coaching, training, corrective action, performance evaluation and termination in order to provide quality care

· May assist the care team with recruiting qualified caregivers

· May act as the backup caregiver providing care in the clients’ home if necessary

· May perform community outreach on behalf of Outreach Health

· Serves as an advocate for and champions the client’s health by being a liaison between the Managed Care Organizations and Outreach to improve client care

· Knows and follows Company policies and procedures with integrity

· Conducts all communications in an honest and professional manner

· Creates an environment of trust, and a culture where everyone feels valued

· Builds relationships with clients by respecting and recognizing their rights; maintaining confidentiality and working tirelessly to earn and keep the trust of our clients

· Performs any other duties as assigned by the Supervisor

Required Skills

· Must have a High School Diploma or GED with a minimum of two years of full-time study at an accredited college experience or 2 years of experience in a supervisory role in a health care facility, agency, or community- based agency for each required year of college

· Excellent time management and strong organizational skills

· Must have knowledge and comply with applicable healthcare laws and statues (local, state, and or federal)

· Skills in early identification of issues, problem solving and interpersonal communication

· Supervisory and decision-making skills, adaptability, and be a positive leader

· Strong communication skills, both written and oral

· Maintain a valid driver’s license and own a legally registered vehicle that holds minimum liability insurance

Must be humble, teachable, and open-minded

Preferred Skills

· Bilingual (English and Spanish)

· Experience working with Electronic Health Records (EHR)

Must respect clients by recognizing their rights; maintaining confidentiality and work tirelessly to earn and keep their trust. Each team member is expected to perform all duties and demonstrate behaviors and attitudes consistent with Outreach Health’s Mission Statement and Core Values.

Pay Range: 35K - 43K
