Job Summary and Qualifications:

The Facility Case Management Director holds the primary responsibility for managing and coordinating departmental activities at the hospital. This role ensures staff compliance with organizational policies and external regulatory agencies while spearheading the integration of patient care and discharge planning processes with hospital departments and external agencies. The Director oversees daily hospital case management operations and is accountable for achieving established outcomes through collaborative engagement with interdisciplinary teams and external stakeholders. The Director embodies executive presence and possesses excellent communication, presentation, and motivational skills. The ideal candidate will have a proven track record of delivering results, attaining goals, and driving initiatives in case management. This role demands strong leadership, trust-building, team-building, and change management skills to foster departmental success and support case management initiatives. The candidate should be adept at managing multiple priorities, working autonomously on projects, and navigating situations of conflict and controversy with diplomacy and tact.

Responsibilities:

• Directing and evaluating departmental operations, including staffing, case management model, use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives.
• Allocating resources effectively to meet productivity and quality goals.
• Ensuring adherence to care coordination and discharge planning processes, including documentation, timeliness, and quality.
• Collaborating with the Facility CFO and Division CM Leader to achieve established goals and expectations.
• Assessing and improving departmental performance through operational process evaluation, data analysis, and implementation of sustainable performance improvement activities.
• Ensuring compliance with policies and standard operating procedures.
• Establishing working relationships with key stakeholders, including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
• Performing other duties as assigned, adhering to the "Code of Conduct" and "Mission and Value Statement."

Qualifications:

• Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
• Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
• 5-7 years of experience in overall acute care Hospital Case Management.
• 2 years of experience in case management leadership.
• Experience in a leadership CM role in a large acute care facility, preferably Trauma Level.
• Polished communication skills and ability to multitask effectively.

Pay: $102,107.00 - $153,088.00 per year
