Agency is seeking a full-time, experienced, Certified Behavioral Health Case Manager Supervisor (CBHCM-S) to provide supervision of the Case Management Department efforts to ensure that participants receive holistic service planning, continuity of care, effective services and on-going advocacy. The CBHCM-S will provide oversight of the documentation to include: intake paperwork, home visits, Biopsychosocials, FARS, CFARS, the comprehensive service plan, and progress notes. The CBHCM-S will ensure that Case Managers act as advocates for participants; link participants with needed services; monitor participant’s progress and provide individualized supports. Case Management services and activities should be provided in community locations which may include: participant’s residences, shelters, hospitals, community resource sites, and/or medical and behavioral health sites.
Responsibilities: Coordinate all Case Management services in a multi-county service region; oversight of documentation and maintenance of persons served records; maintain and allocate adequate resources of well-qualified staff; ensure adequate supervision and evaluation of all case management staff; develop and maintain productive relationships with community based resources.
Education and Experience Requirements:
Current certification by the State of Florida as a CBHCM with ability to become a CBHCMS
Minimum of Bachelor's degree in human services, psychology, social work or a related human services field required along with one full year, full-time or equivalent experience in working with children/adults who have serious mental illness; or a Bachelor's degree from an accredited university or college and three full years, full-time experience working with children/adults with serious mental illness.
Salary is negotiable.
Must have one year of supervision of service delivery, demonstrated leadership ability, excellent writing skills, ability to communicate effectively with diverse groups and individuals, and a thorough knowledge of disease concepts as defined in DSM V.
Must have a valid Florida Driver’s License and a safe driving record.
