Oregon Health & Science University is Hiring a Heart Failure Advanced Practice Provider Near Portland, OR

Department Overview

The appointee shall provide services as assigned by the supervisor in furtherance of the University’s missions and goals of teaching, research, patient care, outreach and public service. The Advanced Heart Failure Advanced Practice Practitioner (HFAPP) will focus on the medical needs of acute and chronic heart failure patients before and after advanced heart failure therapies as a member of the Heart Failure team, primarily in the ambulatory setting. All patients in this clinic were initially referred for consideration of advanced heart failure therapies. In addition, as a member of the OHSU- KCVI team, the HF APP will work in partnership with faculty physicians, fellows, residents, and other APPs to provide excellent clinical care and customer service to our cardiovascular outpatients. The HF APP will participate in relevant educational conferences and be familiar with active cardiovascular treatment protocols.

Function/Duties of Position

Clinical: This role will be 100% focused in the Heart Failure ambulatory practice - with flexibility to meet patient (inpatient) care needs, as required.

  • Evaluation and management of patients with acute and chronic systolic and diastolic heart failure.
  • Outpatient management of chronic and acute heart failure, and palliative management of end stage heart failure.
  • Ambulatory management of patients before and after advanced heart failure therapies.
  • Ambulatory management of patients with chronic heart failure that are not candidates for advanced heart failure therapies.
  • Participation in Heart Failure team conferences, listing meetings, clinic conferences.
  • Assist in generation and maintenance of order sets and other tools to facilitate Heart Failure best practices.
  • Management of patients with heart failure that are living with durable mechanical circulatory support, implantable PA pressor sensors, and/or long-term inotropic therapy.
  • Ability to provide guidance to RN’s working on the heart failure team and provide clinical expertise and mentorship.
  • Ability to work with the broader heart team providers and primary care cardiologists and physicians in a collaborative model to coordinate complex and team based care.

Administrative responsibilities:

  • Participation in Heart Failure team conferences and professional activities.

Education:

  •  OHSU is a teaching institution and as a faculty member, you are expected to provide mentorship and direction to students, residents, fellows and staff.

Administration:

  • Administrative duties may be assigned to you in support of Institute or section initiatives, faculty development and/or other activities as determined necessary.

Required Qualifications

Position Conditions/Qualifications:

  • ACLS and BLS certification required
  • Willingness to obtain additional Heart Failure certification, as required

Nurse Practitioner:

  • Current & Active Nurse Practitioner license for the state of Oregon
  • State of Oregon Nurse Practitioner Prescriptive Privileges
  • Active DEA certificate (upon hire)
  • ANCC (ACNP, AGACNP) Board Certification required to provide inpatient services
  • Membership in professional nursing organization (preferred)

Physician Assistant:

  • Current & Active Physician Assistant license for State of Oregon
  • Active DEA certificate, and Oregon Dispensing Authority (upon hire)
  • Membership in professional organization (preferred)

Experience:

  • 3-5 years prior medical experience in Cardiovascular Medicine, (experience may be as a staff RN)
  • Inpatient and ambulatory work experience preferred
  • Interest and knowledge of chronic disease management
  • Experience in working with heart failure, LVAD and heart transplant patients preferred
