Scope & Objective:

The CNA/HHA will work closely with the Registered Nurse to perform the following duties (based on the Home Health Aide Assignment sheet).

Tasks/Duties & Job Responsibilities:

* Check patients' pulse, temperature and respiration.

* Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).

* Direct patients in simple prescribed exercises and in the use of braces or artificial limbs.

* Maintain records of patient care, condition, progress, and problems in order to report and discuss observations with a supervisor or case manager.

* Massage patients and apply preparations and treatments, such as liniment, alcohol rubs, and heat-lamp stimulation.

* Provide patients with help moving in and out of beds, baths, wheelchairs or automobiles, and with dressing and grooming.

* Change bed linens, wash and iron patients' laundry, and clean patients' quarters.

* Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert.

* Plan, prepare, and serve meals to patients, according to prescribed diets.

* Provide patients and families with emotional support and instruction in areas such as infant care, preparing healthy meals, independent living, and adaptation to disability or illness.

Optimal compensates employees based on a per visit rate.

Qualifications

Qualifications:

* Must be currently licensed and in good standing in the state of Colorado.

* Must be able to communicate in the language of the client and document client related activity as appropriate.

* Possesses adequate and effective written and verbal communication skills.

* Possess adequate and effective interpersonal skills.

* Preferably one year of experience as a CNA / CHHA. Required previous Home Health experience.
