Responsibilities:
Case Management (full-time)
An opportunity to work with individuals with disabilities, the elderly, and their families.
This position requires an individual who would like to work in a very positive, friendly, and supportive environment, serving individuals with disabilities, the elderly, and their caregivers.
The case manager visits program participants in their homes monthly and provides oversight and support to their family caregivers. The case manager will conduct psychosocial and home safety assessments and contribute to the multidisciplinary plan of care. The case manager completes various administrative duties and writes progress notes for each encounter. Training will be provided. The case manager will be in our Waltham office roughly 50% of the time and on the road visiting families for the balance.
Qualifications:
  • Bachelor's degree in any discipline and two years of experience working with elders and/or individuals with a disability
  • Ability to work within an interdisciplinary team environment
  • Current driver's license and dependable car
  • Computer literate
  • Acceptable CORI
Additional Info:
Work Schedule: Monday-Friday 8:00 – 4:00 or 9:00 – 5:00, with some flexibility. No evenings, weekends, or holidays. Mileage for travel to families homes reimbursed at 66 cents per mile.
