Job Purpose:
The LPN Case Manager functions under the direction of, and in coordination with, the Medical Operation Supervisor and/or Provider(s). Participates in the multi-disciplinary process of providing care and treatment to patients. They provide clinical support and technical assistance to the professional medical and other staff. The LPN Case Manager is responsible for functioning within the scope of practice dictated by the authority of the Louisiana State Board of Nursing. This position is also responsible for coordinating and managing patient care based on program requirements. Individual must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with patients and their families.

Required Education
Current LA LPN Louisiana license
Skill, Licensure and Knowledge Requirements
3-5 years case management experience with prior infectious disease and/or behavioral health preferred
Current Basic Life Support (BLS) Certification required
