Open Health Care Clinic is Hiring a LPN Case Manager Near Baton Rouge, LA

Job Purpose:

The LPN Case Manager functions under the direction of, and in coordination with, the Medical Operation Supervisor and/or Provider(s). Participates in the multi-disciplinary process of providing care and treatment to patients. They provide clinical support and technical assistance to the professional medical and other staff. The LPN Case Manager is responsible for functioning within the scope of practice dictated by the authority of the Louisiana State Board of Nursing. This position is also responsible for coordinating and managing patient care based on program requirements. Individual must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with patients and their families.

Essential Job Functions including, but not limited to:

1. Satisfies Core Competencies:

  • Accepts responsibility for ensuring that his/her nursing practice and conduct meet the standards of the professional, ethical and relevant legislated requirements

2. Patient Relations:

  • Achieves customer/patient’s satisfaction by greeting and proactively attending to patient needs.
  • Functions as case manager and advocate for patient population, assisting the patient in navigating the healthcare field.
  • Responds to patient’s requests in a timely and efficient manner.
  • Assists in making patient appointments, arranging transportation, obtain all necessary patient information, and initiatives referrals as appropriate.
  • Maintains confidentiality of patient’s information at work and off duty.

3. Clinical:

  • Performs clinical nursing duties with high integrity, quality and under the clinical supervision of the Medical Operation Supervisor, and in coordination with the clinical care teams.
  • Performs nursing visits which may include, but not be limited to: STI testing and treatment, pregnancy tests, injections, BP checks, urine tox screens, education, TB tests, etc. (as directed by providers).
  • Performs point-of-care lab tests; obtain and process specimens for outside laboratories
  • Determines clients' requirements by completing intake interviews
  • Reviews Providers’ evaluations, treatment objectives, and plans.
  • Plans nursing care following agency policies, standard operating procedures, and standing orders when appropriate.
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
  • Coordinates care and follows up on referrals; evaluates response to treatments; and intervenes in crises.
  • Assures quality of care by adhering to therapeutic standards; measures health outcomes against patient care goals and standards; makes or recommends necessary adjustments; follows hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations
  • Performs medication administration follows best practices
  • Performs in-person and telephone triage for patient’s calling or presenting to clinic using sound judgement and available resources
  • Maintains necessary records and reports of services/program activities as requested by Clinic Quality Committee.

4. Supervisory:

  • Performs annual performance evaluations for staff who report directly to them
  • Assists to organize and assign duties and tasks to clinical team members.
  • Monitors daily performance of clinical team members to ensure duties are completed

accurately, efficiently, and timely

  • Implements or recommends disciplinary action as needed in conjunction with Director of Nursing and/or Human Resources
  • Ensures implementation and compliance with clinical policies and procedures.

5. Patient Education:

  • Provides education related to preventive health topics such as tobacco cessation, nutrition, and physical activity, immunizations, recommended health screenings, sexually transmitted infections (STI’s), etc.
  • Provides education related to medication adherence, appointment compliance, and general health
  • Providers pediatric education related to child safety, developmental milestones, feeding, oral health, etc.

6. Safety/Infection Control:

  • Strictly adheres to Universal Precautions
  • Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
  • Follows job safety requirements. Reports and documents accidents or incidents within 24 hours of occurrence to the immediate supervisor.
  • Organizes exam rooms, including general cleaning, upkeep, and re-stocking of all supplies

7. Workforce Development:

  • Responsible for maintaining up-to-date knowledge, skills, and abilities
  • Attends and participates in in-service, monthly staff meetings, continuing education offerings, community training events, conferences, and webinars as appropriate

Required Education

Current LA LPN Louisiana license

Skill, Licensure and Knowledge Requirements

3-5 years case management experience with prior infectious disease and/or behavioral health preferred

Current Basic Life Support (BLS) Certification required
