GENERAL DESCRIPTION
Under the general direction of an employee of a higher classification, performs professional work in case management services designed to help individuals become self-sufficient. Work involves performing a variety of complex duties concerned with determining eligibility, developing service plans, administering acuity assessments, monitoring progress of individuals, and providing assistance and referrals to housing and social services programs. Work is performed with independent judgment and also involves interpretation of policies and regulations related to all programs and funding requirements. Performs other work as requested.
MINIMUM REQUIREMENTS
EDUCATION AND EXPERIENCE
Bachelor's degree in social work, psychology, sociology, or a related field and one year of case management experience.
SUBSTITUTIONS
Any equivalent combination of education and experience.
CERTIFICATIONS/LICENSES/REGISTRATIONS
