Currently seeking a skilled and driven Social Worker to welcome our ever-growing team!

Our goal for The Rehab of K C Southis to provide our residents with quality care. We desire individuals who have a passion for helping people, who thrive our team environment, and who want to make a difference all while advancing their skills.

Responsibilities:

• Maintain a community resource list
• Complete initial assessments
• Implement care plans
• Ensure regulatory compliance
• Complete MDS sections as assigned, ensure all areas are addressed in thecare plan as necessary.

Requirements:

• A bachelor's degree in social work or a closely related field
• A master's degree a plus
• State licensure preferred (CSW, ACSW, LSW, LCSW)
• Interpersonal, critical thinking, and organizational skills
