Position Description:

The Social Worker reports to the Team Manager for direct supervision. The Social Worker assists patients, families and primary caregivers in adjusting to illness and grief, resolving personal, financial and environmental issues that occur with expected death, and provides grief counseling according to individual family, and primary caregiver needs.

The Ideal Candidate Will Possess the Following Requirements:

• Master’s degree in Social Work required
• Current, unencumbered LMSW license in the states of Kansas and Missouri, required
• Minimum of one year experience in medical social work and/or community home visits, preferred

• Ability to calmly communicate with individuals who are under stress or in crisis

• Good oral, written and interpersonal communication skills; proven social service skills

• Ability to coordinate multiple tasks concurrently
• Knowledge of Google docs, sheets, Microsoft Office, Word, Excel helpful; IT competent
• Good phone and public communication etiquette
• Licensed driver with reliable transportation and auto insurance; must be able to drive anytime during a 24 hour day, if needed
