JOB SUMMARY:

Under the supervision of the DON, the Director of Woundcare Services - RN adheres to the standards of care, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activities. This position is responsible for providing skin care treatments to residents under the medical direction of the resident’s attending physician and per the facility’s skin care program. This position is dedicated to the promotion of good skin care, the healing of existing skin conditions and the prevention of skin breakdown/injury.

DUTIES/RESPONSIBILITIES:

· First and most important responsibility as a leader is to make those who report to you, and your colleagues BETTER.

· Provides skin-related licensed nursing care to residents in accordance with current federal, state, and local standards, guidelines, and regulations.

· Supervises day-to-day Certified Nursing Assistant (C.N.A.) compliance with resident toileting, position, incontinence care, positioning resident devices and general hygiene care.

· Ensures the C.N.A personnel are completing bathing sheets accurately and reporting skin conditions in a timely fashion.

· Monitors and ensures that C.N.A.’s and licensed nurses are providing preventative skin care protocols. Provides training, in-servicing and disciplinary actions as warranted and deemed appropriate.

· Monitors and directs as necessary the provision of non-licensed ancillary department services in the absence of ancillary department supervisors (i.e. dietary services, housekeeping services, laundry services, maintenance services, and security services).

· Participates in C.N.A. nursing staff performance evaluations as directed by ADON and/or DON.

· Disciplines and supervises assigned C.N.A.’s as warranted.

· Completes accurate and timely clinical record documentation per facility policy and State/Federal guidelines and regulations.

· Coordinates all pressure ulcers and wound care needs.

· Follows all treatment orders.

· Prepares weekly skin care reports.

· Assures all skin care sheets are completed weekly per facility protocol. Evaluates, coordinates, and directs appropriate skin care from all levels of nursing services.

· Assesses residents to determine high, medium, or low risk for skin breakdown.

· Develops and assures prevention program is in place.

· Follows up and monitors all residents who require suture/staple removal.

· Provides tracheotomy care daily as ordered by attending physician and as directed by DON.

· Prepares and participates in Skin Care QA Committees and weekly skin meetings.

· Coordinates and works with skin care consultants.

· Participates and develops in-services for all staff regarding treatments, skin prevention program, and early detection.

· Evaluates all patients/residents within 24 hours of admit/re-admit.

· Assesses all residents with Foley catheters to determine necessity and/or ongoing need. In addition, duties include as assigned by DON:

· Assesses g-tube sites

· Assesses Picc line sites

· Changes Foley catheters as ordered by the attending physician and as directed by DON.

· Assists nurses when PRN changes are needed.

· Ensures and/or works with central supply that an adequate supply of floor stock medications, supplies and equipment is on hand to meet the nursing needs of the residents. Reports needed items to the ADON and/or DON.

· Ensures the resident environment is as free from accidents and hazards as is possible. Immediately addresses and corrects potential safety risks as prudent and warranted.

· Communicates information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.

· Ensures compliance with infection control standards. Immediately addresses and corrects identified instances of non-compliance.

· Oversees nutritional program as assigned by DON.

· Prepares and participates in weekly NARS as directed by DON. Including but not limited to documentation and follow-up with RD.

· Ensures all components of the nutritional program and as it relates to wound care and healing are in place.

· Ensures all medication management is in place as it relates to wound care and healing. Including but not limited to wound healing medications and pain management.

· Performs duties, which may include transportation or residents/patients, as assigned in Facility.

· Resident Rights

· Knows the Resident Rights and helps resident understand and exercise their rights.

· Reports resident concerns, complaints, comments to appropriate Administrative personnel.

· Maintains confidentiality of all resident information.

· Safety

· Knows and follows Villa Center Rules.

· Demonstrates proper use of equipment. Reports equipment needs or repairs.

· Follows facility smoking policy.

· Reports and follows appropriate protocol to report any accidents/incidents of residents, staff or visitors to the appropriate Administrative personnel.

· Reports any hazardous conditions/equipment.

· Understands and follows any Disaster Plans.

· Understands and follows infection control standards, policies and procedures.

· HIPAA

· Follows and adheres to Villa’s policies and procedures regarding HIPAA compliance.

· Ensures integrity and confidentiality of all patient financial data.

· Reports all suspected violations of company’s HIPAA policies and procedures to appropriate Administrative personnel.

· Responsible, attentive, knowledgeable and provides high quality care to patients.

· Must be able to adapt to a flexible work schedule and occasional overnight travel.

· Must be available to 24 hours a day on-call emergencies support.

· Possess special interest in, and a positive attitude about, working with short and long-term care residents and the elderly.

· Must be willing to participate in all-hands on dining.

· Possess excellent management skills including the delegation of responsibility to subordinates.

· Ability to read, writes, speak, and understand English.

· Meets all health requirements, as required by law.

· Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Microsoft Office version 1997, 2000 or XP, MS Word, MS Access, MS Excel, E-mail, Outlook, Internet Explorer.

· Perform other duties as assigned and consistent with level of preparation and experience.

EDUCATION and EXPERIENCE:

· Nursing Degree or diploma from an accredited College or University.

· Possess a current license to practice in the State as a Registered Nurse.

· 2 years of supervisory experience preferred.

· Long-Term Care and MDS experience preferred.

ENVIRONMENTAL AND PHYSICAL REQUIREMENTS:

· Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.

· Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.

· Meet all health requirements as needed.

· Dialogue concerning ADA reasonable accommodations are always available upon request – i.e. the interactive process.
