JOB SUMMARY:
The Patient Navigator assists the patient with smooth navigation through the health care process and serves as his/her advocate. Connects patients with resources, health care and support services and assists with the application processes. Develops relationships with service providers and community resources to enhance service and referral networks. Responsible for ongoing Patient Navigator Program development and evaluation. Tracks interventions and outcomes.Works closely with members of the patient’s care team to assure a complete and comprehensive plan of care. Provides clinical support to patients and patient support team.
QUALIFICATIONS:
Education
Master’s or doctoral degree in Social Work from an educational program accredited by the Council on Social Work Education required.
Experience
Experience with screening for common mental health disorders preferred.
Minimum of five years experience in the health care field preferred.
Minimum of two years social work experience in oncology preferred.
Licensure/Credentials
Clinical Social Work license for the State of Washington is required. If not present at time of hire, must be obtained within one year of employment.
Washington State Licensed Independent Clinical Social Worker (LICSW) certification preferred.
Affiliation with the Association of Oncology Social Workers preferred.
