Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a crucial role in coordinating and advocating for the healthcare needs of our patients. Responsibilities:
-Provide case management services to individuals on the ADvantage Program
-Conduct assessments to determine members' needs and develop individualized care plans
-Coordinate and monitor services provided by various agencies
-Advocate for members' rights and ensure they have access to necessary resources and services
-Collaborate with an interdisciplinary team to develop and implement service plans
-Monitor members progress and make adjustments to service plans as needed
-Maintain accurate and up-to-date documentation of all member interactions and services provided
-Review and approve documents from assigned case managers prior to submission to ADvantage for completeness and accuracy
-Supervise and coordinate day-to-day management of case managers and case management activities
- Collaborate with healthcare professionals to develop and implement individualized care plans for patients
- Conduct comprehensive assessments of patients' medical history, current condition, and treatment goals
- Coordinate with hospital staff, including ICU and ER teams, to ensure seamless transitions of care
- Monitor and evaluate patients' progress, adjusting care plans as necessary
- Advocate for patients' needs and rights within the healthcare system
- Facilitate communication between patients, families, and healthcare providers
- Assist with diagnostic evaluations and interpretation of test results
- Ensure accurate documentation of patient information and treatment plans
Skills:
- Registered Nurse (RN) license recommended but not required
- Strong knowledge of acute care practices and procedures
- Proficiency in medical terminology and physiology
- Experience working in a hospital or similar healthcare setting
- Familiarity with medical records systems and electronic health records (EHR)
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Strong organizational skills with attention to detail
- Knowledge of managed care principles and practices
