Provide on-site case management services, support groups, and socialization opportunities for individuals who are pregnant or who have recently been pregnant, and their families. Link families with basic needs, physical health appointments, mental health appointments, housing issues, and education and employment opportunities. Including arranging, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way. Provide a single point of contact and work in partnership with others as part of a multidisciplinary team to provide a set of social service functions that helps clients access the resources they need to recover.
Responsible for Oklahoma Department of Mental Health and Substance Abuse Services and Medicaid billing.
