Bachelor-level starting pay $18.00 an hour with $1000 sign-on bonus

What You’ll Do Case Managers in Crisis Services provide crisis services through the walk-in, hotline and mobile response. Case Managers will help adults in our community live healthier and more fulfilling lives through identifying and supporting their needs and goals. Case Managers specialize in:
Managing the care provided to adults
Helping adults gain access to the services they need to meet their goals
External collaboration with community partners
Provide crisis services
Primary duties for case managers include:
Evaluating the need for services
Developing and monitoring a treatment plan that supports the person’s goals
Helping obtain services needed to support goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
Advocating and serving as a link between healthcare providers, employers, community agencies and other Oaklawn services
Documenting all services into the electronic record
Providing client care based on level of need
Interacting frequently with the person’s treatment team including outpatient therapists, physicians, other treatment providers involved, community agencies, referral sources and other services.
Caseload expectations will take into account training, case complexity, level of frequency and/or other assigned duties
Hours may include availability in the evenings and/or weekends
Minimum Hiring Requirements for case managers:
BA/BS in social service or a human services-related field required
Two years’ experience in a social service setting preferred
Must have a valid driver’s license and proof of insurance
