Guaranteed minimum hours and pay (weekly pay depending on travel vs local)

Additional $.45 cents per mile for patient related travel

Pay will be based on experience and skill sets.

• Minimum 1 years as PT required
• Minimum 6 months Home Health experience preferred
• Charting system is HCHB (Homecare Homebase) - required experience
• On call rotations determined by hospice / home health agency 
• To follow local governing board scope of practice.
