General Statement of Duties:Caseworkers in the Office of Community Services provide casework and advocacy services to assist clients in resolving economic challenges. Employees in this class work with individuals, families and children in the community including persons experiencing homelessness, persons with disabilities or chronic illnesses, elderly, persons with mentally illness, substance dependence, and ex-offenders. Caseworkers use assessment tools to measure client strengths, needs and assess for risks. Caseworkers are responsible for preparing, monitoring and evaluating individual service plans and providing interventions to facilitate alternative solutions to specific problems. Caseworkers prepare statistical information for local, state and federal reporting requirements. In addition, Caseworkers establish and maintain interagency relationships and community partnerships for provision of services and advocacy.

Examples of Duties:
* Provides direct client services to households experiencing an economic crisis:
o Conducts interviews utilizing counseling, crisis intervention, and problem solving techniques to engage and establish rapport with clients;
o Conducts biopsychosocial assessments to determine immediate needs, treatment or service history, strengths, and barriers to stability such as employment, housing, childcare, physical and mental health issues, substance use, and criminal history;
o Assesses initial and on-going risk and level of need using evidenced based assessment tools;
o Reviews verifications, prepares case narratives, and makes recommendations for financial assistance based on federal, state and local program guidelines;
o Provides emergency walk-in coverage for clients presenting with immediate needs or crises;
o Develops individual service plans addressing service needs in life domains, monitors client progress and provides supportive services to ensure individual service plan goals and strategies meet changing needs and priorities;
o Refers, coordinates and monitors the delivery of appropriate services and leverages community resources to meet clients' basic and treatment needs;
o Conducts home visits and accompanies clients to appointments as needed; Conducts outreach in the community to provide services to clients in setting most comfortable for them;
o Assesses immediate risks to health and safety and follows procedures to ensure clients' and families' welfare, including making referrals to Child Protective Services staff or Adult Protective Services staff when warranted;
* Prepares statistical data and reports:
o Maintains confidential and comprehensive client records, contact notes, social history narratives and service plans in secure case management database;
o Prepares monthly and quarterly statistical reports according to federal, state, and local and/or grant requirements;
o Updates and maintains contacts, writes reports and quarterly narratives of progress;
o Uses data to assist in identifying gaps in services and providing innovative solutions;
* Works closely with community partners advocating and educating community on homelessness and poverty issues:
o Convenes multidisciplinary case consultation team meetings to aid in identifying problems, service needs and engagement techniques;
o Maintains communications with various agencies, service providers, and community partners, and acts as a liaison to advocate for clients within all systems;
o Provides guidance and training to graduate interns, community volunteers and new staff;
o Provides guidance to departmental and City agency staff and community organizations on prevention and intervention programs;
o Performs related work as required, accepts special projects and other duties as assigned.
Essential Knowledge Skills and Abilities:
Knowledge of the principles and practices of case management; experience providing case management services and applying social work techniques and practices; knowledge of social, psychiatric, economic and health problems; knowledge of early childhood development theory, principles and practices; ability to understand laws and interpret and apply policies and procedures; knowledge and ability to apply casework and counseling techniques for clients with mental and substance disorders, alcoholism, and trauma survivors; knowledge of community resources or the ability to readily acquire such knowledge; experience working with low-income culturally diverse population facing crisis; ability to quickly assess and defuse hostile situations; ability to identify social problems and needs, and to assess the strengths and challenges of individuals and families and children; ability to use strengths based approach in work with clients; ability to plan, prioritize and organize multiple assignments; ability to communicate effectively, both verbally and in writing; ability to provide guidance and assistance to new staff, interns and volunteers; ability to establish and maintain effective collaborative working relationships with clients, co-workers, professionals, and the general public; possess proficient computer skills and ability to prepare and maintain complete and accurate records.

Minimum Qualifications:

Preferred Qualifications:
Bachelor's Degree in social work or related field and at least three years of experience in provision of case management social work and counseling services to vulnerable populations. Experience working with clients in crisis and engaging non-compliant individuals in services. Extensive knowledge of social, economic, health, and behavioral problems; ability to identify social problems and needs in order to assess clients' and/or their family's ability to utilize available services; strong knowledge of benefit programs and community resources and ability to interpret laws, policies and procedures, and regulations for such programs; ability to establish and maintain effective working relationships and advocate for service population and liaison with community resources; ability to communicate clearly and effectively both orally and in writing; ability to prepare and maintain complete and accurate records and reports and possess proficient computer skills. Bilingual skills (English/Spanish) preferred.
