Northern Manhattan Perinatal Partnership (NMPP) a maternal and child health agency located in Central Harlem seeks three Community Health Worker (CHW) for its Perinatal and Infant Community Health Collaboration (PICHC) program. The CHW will engage and enroll women of ages 14-44 focused on pregnancy, reproductive health and health insurance enrollment, health education, case management, and advocacy services to make sure they are connected to quality services to address their needs to assist with healthy birth outcomes.
RESPONSIBILITES:
1. Conduct outreach in the neighborhood to recruit women.
2. Conduct at least one monthly home visit to assess need for other services and to provide appropriate health education sessions.
3. Provide the following services based on assessment and action plan: case management services, advocacy, and health education services.
4. Provide escort/advocacy to clients to medical appointments, government agencies as needed.
5. Conduct presentations and educational workshops in-house and in the community about the program
6. Work with service providers to ensure services for clients are coordinated.
7.Maintain up-to-date case records documentation and database reflecting client contact service plans accomplishments
8. Represent the program at other agencies
9. Participate in collaboration activities internally and externally related to program goals
10. Perform other duties as necessary
QUALIFICATIONS
* Associates Degree in Human Services, Community Health Education, or any closely related field. CHW certification a plus but not necessary.
* Bilingual, fluent in English/ Spanish
* Experience and willingness to work with women and their families in the Lower Manhattan areas
* Ability and willingness to conduct fieldwork such as home visits, outreach etc.
* Ability to work with a team and independently
* Ability to find community resources
* Excellent engagement and communication skills
* Computer and internet literate
