Medical Social Worker – PRN

Northern Homecare is looking for a Medical Social Worker (MSW) to serve our homebound and Hospice patients on an intermittent basis. This service is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. This position is in-person on a PRN basis. We are looking for a compassionate, caring, and patient individual to join our team.

Our territory includes Manistee, Benzi, and/or Grand Traverse counties.

Requirements:

• Master's Degree from an accredited School of Social Work.
• Baccalaureate degree in social work, psychology, sociology, or other field related to social work, and has had at least one year of social work experience in a health care setting.
• 2 years’ of appropriate experience.
• One (1) year experience in home care preferred.
• Analytical thinking, and problem-solving abilities. Includes home visits to private homes. Only act within the scope of his or her authority to practice.

Key Responsibilities:

• Understands and promotes principles of QAPI.
• Maintains current knowledge of Medicare Conditions of Participation for social work services.
• Refers patients and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants.
• Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.
• Participates in in-services and/or continuing education programs.
• Participates in patient care conferences.
• Documents patient changes appropriately in the medical record.
• Plans/coordinates all social services within the Agency.

Compensation: $75.00 - $85.00 per visit
