This is a dynamic position working requires an individual with housing and case management knowledge.
*Responsibilities of the Housing Case Manager: *
* Manage a caseload of specific clients including developing Service Goal Plan, budgeting, and supporting clients’ successes in achieving short- and long-term goals
* Facilitate support-educational groups to increase client knowledge of relevant life skills and community resources including, but not limited to, basics of renting, fair housing, self-esteem, safety, parenting, child care, nutrition, money management, credit counseling, home maintenance, and good housekeeping skills
* Make referrals to other services as needed and work with other community providers to provide collaborative services to clients
* Conduct unit inspections to ensure compliance with safety and federal/funder regulations
* Conduct and document home visits to document progress and ensure that clients continue to meet program guidelines and eligibility
* Assesses clients in crisis and resolves conflicts. Develops interventions to resolve problematic behavior of clients
* Process re-certifications in compliance with funder requirements, if applicable
* Outreach to landlords for mediation, negotiation, and tenant relations
* Monitor direct assistance that can be used for financial support, rent, utility, and security deposit payments
* Compile and prepare appropriate tenant/landlord contracts and landlord check request payments to ensure timely delivery
* Responsible for building and maintaining relationships with other service providers, including coordinating client service delivery with on-site providers of primary and behavioral health care
*Qualifications of the Housing Case Manager: *
* Bachelor’s degree in social work or a human services field
* 1 year of previous direct support, case management, and administrative support experience
* Hands-on experience in social and human services field dealing with aging populations
* Knowledge of principles, methods and procedures of case management, housing first, and trauma informed care
* Demonstrated ability to develop and maintain timely, accurate and complete documentation
* Housing Counselor and/or HQS (Housing Quality Standards) Certification a plus but not required
* Proficiency in usage of basic technological tools including laptops/computers, email, phones, and internet
* Spanish language fluency preferred but not required
* Home office space and high-speed internet to perform administrative tasks
* Personal vehicle to travel as needed for on site visits and meetings (25%)
