What you will own
Provide comprehensive case management services tailored to individuals and families, focusing on strengths, recovery, trauma awareness, and cultural sensitivity. Conduct thorough assessments of clients' needs, coordinate services, including referrals, accompany clients to various appointments, and offer housing support services. Ensure seamless coordination among service providers and maintain engagement with clients facing challenges in staying connected to care. Collaborate with the treatment team, lead group interventions, accurately document client interactions, liaise with relevant agencies, conduct necessary tests, manage schedules efficiently, and contribute to meetings. Offer crisis intervention, assist with transportation needs, adhere to the Medicaid Compliance Plan, report violations, and support other programmatic tasks as required.

- Strong interpersonal communication (written and spoken)

- Adept in Microsoft Suite and able to quickly learn relevant applications
Non-negotiables
- Bachelor's degree in social work or related field.
- Minimum 2 years of experience providing direct behavioral health services.
- Bi-lingual/Bi-cultural preferred.
- Ability to work effectively in collaboration with diverse groups of people
