Job Type: Contract
Salary: $50.19 - $58.00 per hour
Location:-Western NY, Queens, North Country, Manhattan, Hudson Valley, Central NY , Brooklyn and Bronx .
Expected hours: 40 per week
REQUIREMENTS:
- Two (2) year's of experience in a Home setting within the last ten (10) year's.
- Working in the field of Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC).
- Conducting assessments that are similar to the UAS such as OASIS.
- Providing primary care to homebound persons.
- Care Management for persons receiving home and community-based services including Hospice & Assisted Living.
- Home Care experience as a paraprofessional # RN3
Job Type: Contract
Pay: $50.00 - $58.00 per hour
Expected hours: 40 per week
