Summary/Objective

The Therapy Services Department provides physical, occupational, and speech therapy services to all residents at New Vista Hazelwood. The Therapy Services Department also works closely with the Nutritional Department in the development of programs. Staff in the department complete discipline specific assessments, perform treatments, develop mealtime and physical support plans, train staff and monitor programs. All services are provided at the highest standard of professional practice, complying with standards by both federal and state regulations and facility policies.

Required Education and Experience

  1. Graduate of an accredited Occupational Therapy Assistant program.
  2. Must be licensed by the Kentucky Board of Licensure and Certification for Occupational Therapy
  3. Must be licensed by the National Board of Occupational Therapy.

Preferred Education and Experience

1. Preferred experience working with individuals with intellectual disabilities.

Position Type/Expected Hours of Work

This is a full-time position, days and hours of work are typically Monday through Friday, 7:30-5:30. Therapy assistants are expected to flex their hours to meet the individual and facility needs. This is an hourly 37.5-hour work week.

Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Implements treatment plan of care at the direction of the occupational therapist.
  2. Monitor and report status and progress of individuals.
  3. Complete all required documentation.
  4. Train and monitor physical and sensory support plans.
  5. Provide staff training as assigned.
  6. Participate in interdisciplinary team meetings and other meetings as assigned.
  7. Participate in new employee orientation as needed.
  8. Perform other duties as assigned by Therapy Director or Assistant Therapy Director
  9. Shall provide services to meet the treatment needs of the specialty intermediate care (IC) clinic patients for services in field of care. Shall meet requirements of 902 KAR 20:410 Specialty Intermediate Care Clinics as warranted.
  10. Comply with all state occupational therapy licensing requirements and regulations.
  11. Complete all required inpatient and outpatient billing.
  12. Learn and follow all policies and procedures.

Competencies

  1. Requirements for state licensure (minimum of 12 CEUs per licensure cycle)
  2. OJT Competencies for new employees

Supervisory Responsibility

Supervises therapy support staff when needed.

Travel

This position may require travel to the Group Homes locations as well as community programs outside of Hazelwood Center when an individual is transitioning (i.e. SCL, day program, FHP, etc…). Travel is less than 20% of the expected total job duty.
