We are looking for a Case manager to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s human service needs. A certified case manager is a plus.

Responsibilities

• Assisting individuals and families to access essential and needed resources, i.e., medical, social, educational, etc.
• Conduct comprehensive assessments of individual strengths and needs.
• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
• Handle case assignments, draft service plans, review case progress and determine case closure.
• Help clients achieve wellness and self-sufficiency.
• Facilitate multiple care aspects (case coordination, information sharing, etc).
• Collaborate with NHCR providers to develop an individualized care plan to include the client's family and any relevant agencies.
• Arrange coordination with social services, health and governmental agencies.
• Maintain a case load of 25 clients.
• Meet all TCM regulations regarding number of monthly encounters.
• Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction.
• Record cases information and complete accurately all necessary forms.
• Promote quality and cost-effective interventions and outcomes.
• Adhere to professional standards as outlined by protocols, rules and regulations.

• Current license, certification or registration preferred, but not required upon hire.
• BS degree in related field (Social work, human services, etc.)

Job Types: Full-time, Contract

Pay: $20.00 per hour

Expected hours: 40 per week
