Position Title: SSVF Program Coordinator

Department: Human Services

Job Summary:

The SSVF Program Coordinator will have a comprehensive understanding of the SSVF Program’s policies, procedures and all related services and will take the lead on the SSVF Housing Navigation service, Health Care Navigation service and the Supplemental Services Funding. This position will supervise the SSVF Housing Navigator and the Health Care Navigator and will support SSVF team members who are working with Veterans who have a HUD-VASH Voucher to ensure continuity of housing search services and SSVF temporary financial assistance. This position will provide SSVF team members with additional training and support to ensure that Housing and Health Care Navigation services are being fully utilized and are embedded in the SSVF program model. This position will also provide direct Housing Navigation or Health Care Navigation services to Veterans, as needed.

This position employs a variety of evidence-based interventions including Motivational Interviewing, Housing First, Rapid Re-housing, Harm Reduction, and Trauma Informed Care to provide client centered services to Veterans.

Responsibilities:

  • Work closely with the Director of Housing and Stabilization Services and the SSVF Program Managers on the design and implementation of the specific SSVF services of Housing Navigation and Health Care Navigation.
  • Regularly case conference Veteran families with HUD-VASH Case Managers from the Boston, Bedford, and Providence VA Medical Centers to coordinate Housing Navigation services to expedite permanent housing placements, including providing Veterans with the appropriate level of services, and temporary financial assistance.
  • Oversee the Health Care Navigator and services provided, to ensure assignment and follow up for Veterans enrolled in Health Care Navigation. Monitor the referral process for the Health Care Navigation to ensure timely engagement and enrollment.
  • Work closely with the SSVF Stabilization Services Program Manager and the SSVF Rapid Re-housing Program Manager to create a landlord pool and respond to all landlord questions and concerns in a timely manner and develop strategies to engage landlords with incentives.
  • Coordinate services, home visits, and provide referrals to community resources and benefits.
  • Assist Director of Housing and Stabilization Services with implementing current policies, procedures and forms that facilitate the effective operation of the SSVF Program.
  • Act as the lead for SSVF Supplemental Services, including client and landlord incentives. Ensure that temporary financial assistance documentation is accurate and submitted to Finance in a timely manner.
  • Review and audit program documentation, including service notes, Integrated Service Plans, documentation of Housing Navigation and Health Care Navigation services and other required SSVF documentation to ensure compliance with funding requirements, program model and deadlines.
  • Intervene and/or provide guidance during crisis situations as needed.
  • Attend Program, Department meetings, CoC meetings, VASH meetings, SSVF Regional meetings and trainings.
  • Perform other SSVF duties as assigned.

Qualifications


Required Qualifications

  • Bachelor’s degree required.
  • Supervisory and program implementation experience.
  • One to three years’ experience with Veteran population, mental health and substance use disorders, subsidized housing and/or homelessness.
  • Understanding of and commitment to Housing First, Harm Reduction, Motivational Interviewing, and Trauma Informed Care.
  • Strong case management and housing navigation skills and understanding of community based resources and how to proficiently navigate these resources.
  • Familiarity with Housing Authorities’ procedures, including inspections and Requests for Tenancy Approval.
  • Excellent customer service skills and the ability to communicate professionally.
  • Proficiency with electronic databases and computers.
  • Strong crisis intervention skills.
  • Flexible self-starter with strong analytical skills.

Preferred Qualifications

  • Knowledge of military culture.
  • Knowledge of Massachusetts real estate.

  • Ability to work comfortably with a multicultural, homeless population experiencing complex challenges.
  • Demonstrate positive communication with Veterans and other team members and collaborators.
  • Strong organization skills and attention to detail.
  • Strong computer, database and electronic recording keeping skills.
