ROLE OVERVIEW and PURPOSE

The Occupational Therapist works to support the mission and vision of Neighborhood Healthcare by providing occupational therapy services for patients to remain independent and live safely in their communities. This role will design and develop patient care plans based on mobility assessments. In addition, this role will supervise the support team to ensure patient care plans are carried out correctly to increase success rates for patients.

RESPONSIBILITIES

  • Develops and implements occupational therapy programs for patients accessing day services
  • Performs assessments of patients to determine physical abilities and mobility
  • Develops and implements individualized plans of care to decrease or eliminate disability, increase, or maintain capability for independence, and develop function to a maximum level
  • Monitors qualified support staff activities to ensure individual plans of care are followed appropriately
  • Guides patients and caregivers in the use of adaptive devices and therapeutics and creative and self-care activities for improving function
  • Evaluates the effectiveness of treatment plans and modifies goals, as needed
  • Conducts home, hospital, and other site visits as indicated by IDT and Center Manager
  • Records and signs treatment plans in the patient's health record
  • Records, signs, and dates quarterly progress notes; including reviews and initials all evaluations and discharge summaries
  • Ensures services provided are in a therapeutic setting and conducive to restoring dignity and self-esteem to patients
  • Manages required equipment and supplies for the occupational therapy program to ensure they are available for patients; order equipment as needed
  • Follows policies which define the occupational therapy responsibilities as a resource to support staff
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)
  • Participates in training programs for staff and caregivers
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops

EDUCATION/EXPERIENCE

  • Master’s degree in occupational therapy and two years’ occupational therapy experience required
  • Valid CA Occupational Therapist license in good standing required
  • One year experience working with a frail or elderly population required

ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with principles and methods for treatment and rehabilitation of functional impairments
  • Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team
  • Knowledge and understanding of the Older Americans Act, Americans with Disabilities Act, and Area Agencies on Aging
  • Ability to apply ingenuity and imagination in adapting activities to individual needs
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 25 lbs/weight

Ability to stand for long periods of time

Pay range: $45.90 - $56.10 per hour, depending on experience.
