ROLE OVERVIEW and PURPOSE
The Transgender and HIV Care Physician Assistant/Nurse Practitioner, will work to support the mission and vision of Neighborhood Healthcare (Neighborhood) by providing excellent patient-centered care and services within a dynamic team environment.Thie role will play a pivotal role in delivering comprehensive patient centered care, fostering a supportive and affirming environment, and promoting overall health and well-being to patients.Additionally, this role will provide direct healthcare services to patients under the direction and responsibility of physicians.
RESPONSIBILITIES
Direct Patient Care:
  • Performs comprehensive physical examinations on patients, including review of patient medical data, such as health history
  • Orders, administers, and interprets test results for diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests
  • Performs therapeutic procedures such as injections, immunizations, suturing and wound care, and managing infection
  • Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance
  • Impacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracy
  • Assesses and provides age-appropriate care to patients
Transgender Patient Care:
  • Provides competent and culturally sensitive primary care services to patients
  • Offers hormone therapy management, including prescribing and monitoring hormone regimens
  • Addresses and manages the physical, emotional, and mental health needs of transgender individuals
  • Conducts assessments for gender affirming surgeries and facilitate appropriate referrals, as needed
  • Works with the behavioral health team by assisting with gender identity exploration and provides support throughout the transitioning process
HIV Patient Care
  • Offers specialized care to patients living with HIV, including antiretroviral therapy management.
  • Monitors disease progression, immune statuses, and potential complications related to HIV
  • Provides counseling and education on adherence to treatment plans and preventative measures
  • Collaborates with interdisciplinary teams to develop and implement comprehensive care plans
Team Support:
  • Shares accountability for overall patient health outcomes, working in coordination with care teams
  • Operates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders
  • Responds promptly and with caring actions to patients and employees
  • Maintains professional working relationships with all levels of staff, clients, and the public
  • Cooperates in accomplishing communicated department goals and objectives
Clinical Administration:
  • Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care
  • Performs in accordance with Neighborhood practice protocols and under the guidance and supervision of supervising providers according to the terms of the Supervisory Agreement
  • Keeps informed of organizational activities and promote mission and goals
EDUCATION/EXPERIENCE
  • Physician Assistant (PA)/Nurse Practitioner, program completion required
  • Valid CA Physician Assistant/Nurse Practitioner, License in good standing required
  • Valid DEA and CPR certifications required
  • WPATH Training required
  • AAHIVS HIV Specialist Certification required
  • Experience working with transgender patients and/or those living with HIV preferred
ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)
  • Excellent verbal and written communication skills to establish rapport with patients and interdisciplinary teams, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with providing direct care to patients
  • Ability to commit to diversity/inclusion and providing patient centered, non-discriminatory and culturally competent care
  • Knowledgeable about and experience with unique healthcare needs and challenges faced by transgender individuals and those living with HIV
  • Knowledgeable about and experience with gender affirming care and HIV treatment protocols
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner
Physical Requirements
  • Ability to lift/carry 20 lbs./weight
  • Ability to stand for long periods of time
Pay range: $147K - 199K per year, depending on experience. (DOE)
