Job Description

The Community Health Worker provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous peoples residing in Maricopa County. Additionally, is a liaison between, the NATIVE HEALTH Community Health and Wellness Department, Medical Department, and community members to facilitate access to services and improve the quality-of-service delivery. Including the coordination of services to improve diabetes-related health outcomes.

Key Accountabilities:

  1. Provide Diabetes-Related Education - Offer education and support to participants through facilitating group classes with topics focused on diabetes management and prevention, hypertension, weight management, healthy eating, stress management, and other chronic diseases via the NATIVE HEALTH newsletter and other written formats, in-person/virtual workshops, and videos.
  2. Community Health Outreach - Provide diabetes prevention education to Indigenous peoples, perform blood pressure checks and blood glucose testing. These events are both external and internal and will include some nights and weekends. Additionally, outreach to members who are historically hard to engage or not linked to community services and provide proactive follow-up and monitoring.
  3. Communication - Maintains consistent and timely communication with direct supervisor, NATIVE HEALTH Senior Management as appropriate, as well as community members enrolled in the Wellness Warrior program via monthly newsletter and email. Communicates and recruits’ individuals and families for program events and workshops. Also promotes clear communication amongst the care team to ensure patient care plans.
  4. Program Development - Assist with planning, program design and assures deliverables are met and reported in a timely manner in collaboration with the Indigenous Wellness Manager and Garden Coordinator.
  5. NATIVE HEALTH Indigenous Wellness Camp - Assists in planning and implementing the annual Wellness Camp. Maintains materials of evaluation both electronic and paper filing system, as well as providing follow up as needed. Provide obesity prevention education and activities while at camp. Must attend five-day, four-night camp once a year.
  6. Records Management - Maintains updated patient/client database of recruitment forms/information and materials of enrollment both electronic and paper filing system, as well as maintain records of services provided to all program participants and equipment inventories.
  7. Information Preparation - Taking meeting minutes, compiling data for reports, creating digital presentations, writing reports, editing, proofreading, and other information preparation duties. Maintains accurate records for the program. Interprets statistical data and provide recommendations for improvement and/or sustainability.
  8. Garden Space  - Assists with garden maintenance, seasonal grow out plans, acquiring supplies, harvest scheduling, and produce transport and delivery to NATIVE HEALTH locations for distribution. Works closely with Garden Coordinator to help with garden workshops including directing volunteer activities.
  9. Patient Care - CHW is responsible for both clinical and administrative tasks, including closing the loop with patients, engaging patients and re-engaging assigned patients, and work as part of the diabetic care team and supports providers through an integrated approach to care coordination and outreach.
  10. Supports the organization as needed.

Qualifications

QUALIFICATION/EXPERIENCE/REQUIREMENTS:

  1. A Bachelor’s Degree in a health-related field is required.
  2. Experience in community outreach or health education is a plus.
  3. Knowledge of Electronic Health Records (EHR) preferred, NEXTGEN a plus.
  4. Must be able to work irregular hours to include evenings and weekends as needed.
  5. Must possess two years of specialized experience working within the American Indian/Alaska Native community.
  6. Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out of state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver’s license during employment. Must be insurable by NATIVE HEALTH liability auto policy.
  7. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment.
  8. Must pass substance abuse testing upon employment and submit to random testing during employment.
  9. Must have an updated Immunization (IZ) Record.
  10. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
  11. Must obtain a CPR card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.

KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Facilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them. 
  2. Understand how social determinants of health impact the community and participants.
  3. Knowledge of Arizona Indigenous tribes’ practices of farming, gardening, and traditional foods.
  4. Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPAA).
  5. Ability to pass blood pressure and blood glucose testing competencies.
  6. Strong problem-solving skills and assertive in challenging situations.
  7. Highly resourceful team-player, with the ability to also be effective independently.
  8. Creative thinker, who actively seeks opportunities and proposes solutions.
  9. Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.
  10. Strong interpersonal skills including tact and flexibility to work effectively with supervisor, staff, and participants.
  11. Advanced computer skills included MS office, specifically EXCEL, PowerPoint, Outlook, and Word.
  12. Develop creative ideas, manage, and deliver diabetes-related education via digital content using a range of techniques, across channels including websites, social media, and Zoom platform.
  13. Culturally competent; able to work with a diverse workforce and client population.
