Montgomery County Government is Hiring a Community Services Aide III (Healthcare for the Homeless), Grade 18 Near Rockville, MD

About the Position:

This Community Services Aide III is located in the Department’s Services to End and Prevent Homelessness division, Healthcare for the Homeless. The position provides on-site assistance to homeless clients primarily in emergency and transitional shelters in Montgomery County and supports the Community Outreach team with unsheltered clients. Primary areas of focus includes: assist with case management of medical bed clients in both male and female emergency shelters; provides supportive resources and communication as the liaison for complex cases on inpatient hospital units for newly identified homeless individuals, provide navigation assistance to clients in the Housing Initiative Program (HIP); vital signs and other task delegated by assigned community health nurse; and provide supportive assistance to the assigned community health nurse in the Housing Initiative Program (HIP).

Duties include:
  • assisting clients in gaining access to and navigating a primary health care medical home and other community based social services (e.g. behavioral health, housing, legal, etc.)
  • health education and community outreach
  • follow up with clients regarding appointments with primary and specialty care, care plans and health goals initiated by assigned community health nurse or discharge agency prior to entering the program
  • provide guidance regarding effective strategies to promote positive behavioral health and discourage risky behaviors
  • visit clients as need determines on a weekly or more frequent basis
  • support colleagues across the program area, including nurses, hospital discharge planners, and other medical personnel; serve as a liaison between clients and their health/community providers
  • provide culturally and linguistically appropriate health or nutrition education to unsheltered and Housing Initiative Program (HIP) clients
  • enhance the client’s ability to appropriately communicate with their healthcare providers
  • complete case notes and reports in a timely manner; assist with coordination of transportation to appointments, which may include accompanying client as needed
  • provide motivational interviewing and goal setting with clients
  • provide medical support to cases assigned to program Community Health Nurses
  • manage the medical supplies for the Health Care for the Homeless Team, support the supervisor with ordering of needed medical inventory, assist with the McKesson/Medical Supply vendor orders and support Nextgen efforts (County EHR)

Minimum Qualifications


Experience
: Three (3) years of experience providing delivery of social, health and/or public assistance services and working in a medical setting.

Education: Completion of high school or High School Certificate of completion recognized in the State of Maryland

Equivalency: An equivalent combination of education and experience may be substituted.

LICENSE:
o Selected individual positions in this class assigned on a regular basis to perform acts of registered or licensed practical nursing as delegated by a Registered Nurse must possess a current Maryland Certified Nursing Assistant certification (CNA).
o Possession and maintenance at all times of a valid State of Maryland driver’s license or state of residency

Preferred Criteria


There is no Preferred Criteria
. All applicants will be reviewed by OHR for minimum qualifications. Those applicants who meet minimum qualifications will be rated Qualified, placed on the Eligible List, and may be considered for an interview.

Preference for interviews will be given to applicants with experience in the following:

  • Strong interpersonal and social skills with demonstrated ability to collaborate with a variety of individuals from a wide range of professional and personal backgrounds.
  • Bilingual English and Spanish
  • Previous experience providing health services to vulnerable populations (i.e. homeless, uninsured, chronically ill, elderly)
  • Experience working with individuals with multiple complex medical disorders (i.e. wound care, chronic diseases like hypertension & diabetes, COPD, cellulitis, assistance with management of medications), and/or untreated behavioral health problems (i.e. depression, bipolar, schizophrenia)
  • Certification in the health field as a Licensed Practical Nurse (LPN) and experience as a health educator
