Description
This is an entry level professional position in community based psychosocial rehabilitation services for individuals with the most severe mental illness. The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Case Managers work independently and must demonstrate excellent clinical judgment, decision making and safety consciousness to assure safety and appropriate treatment for consumers in the community. This role works independently without regular supervision in community settings alone with consumers or leading therapeutic groups.
