Job Description:
PCP1
Less than 2 years’ experience in Home Care. Focusing on homemaking tasks and employees that have no desire to perform Personal Care or Health Maintenance tasks. Will assist clients with homemaking, encompassing light housekeeping, meal preparation, laundry and shopping.
PCP2
2-5 years’ experience in Home Care. PCP’s will provide proficient homemaking tasks as well as Personal Care. Responsible for aiding clients with personal care including, but not limited to, bathing, dressing, eating, transferring, walking, toileting, continence care, medication reminders, hygiene assistance, as well as homemaking tasks.
PCP3
3-5 years’ experience in Home Care or those that let their Certified Nursing Credentials expire. These employees will provide homemaking, personal care, and health maintenance tasks, under the supervision of our Registered Nurse. Responsible for assisting clients with health maintenance activities such as transfers using adaptive equipment, assist with ostomy and catheter care, wound care, respiratory care, medical management, exercise and mobility. Will also aid clients with Personal Care including but not limited to bathing, dressing, eating, transferring, walking, toileting, continence care, medication reminders and hygiene assistance.

-Valid Driver’s License
-Proof of Car Insurance
-Drug Test Screening
-SSN Card
-Vaccination Card (if applicable, but not required)
-Demonstrates proficiency at reading and understanding a clients care plan
-Prior experience in a skilled nursing or assisted living community is preferred
-Must be dependable and capable of pleasantly and tactfully communicating with clients, family staff and other agencies.
-Open to working nights and weekends.
