Summary:

This is a professional position that works as a member of a team with program staff and community resources to provide a continuum of services to children and families. This person strives to strengthen and preserve families. The person in this position assists in identifying individual needs and providing and coordinating services to meet each family's needs. This person works under the supervision of the Family Community Partnership Specialist.

Duties and Responsibilities:

  • Input designated data into computer programs
  • Through the Family Service Assessment and Family Partnership Agreement, engage parents in identifying their needs, developing goals and providing referrals and follow-up
  • Upon registering, inform parents/guardians of all program requirements
  • Advocate for children and families by assisting in the identification and coordination of support services
  • Provide support and crisis intervention with families and children involved with the program
  • Plan and implement activities that teach parents/guardians child development/child management, nutrition education, health education, dental education, mental health education, use of community resources and other adult education activities
  • Conduct and track children's screening status and follow up accordingly
  • Intervene and contact parents when there are problems with attendance
  • Conduct home visits to provide support and/or meet families identified needs
  • Work in a collaborative effort with agency/community resources to provide wrap around and follow up services to families in order to strengthen and enable them to function independently, to improve their capacity to function as a healthy families, or if needed, to find appropriate alternatives for the care of the children

Minimum Qualifications:

  • BA/BS degree from four-year college or university in family support, psychology, sociology, family life, social work, early childhood, child development, human services, family services, counseling, or related field
  • If degree is in early childhood or child development, must have at least 9 college credits in family support, psychology, sociology, social work or family life
  • Minimum of one (1) year prior human service experience working with low-income children and families preferred
