Mint Health NY is a premier group of medical providers who are committed to establishing comprehensive and consistent healthcare for homebound and elderly patients.

We are currently seeking a dedicated and compassionate Behavioral Health Specialist to join our team. This position offers a unique opportunity to make a meaningful impact on the health and well-being of our homebound patients by providing counseling to elderly patients who may be having a hard time dealing with their conditions.

• Educate patients and their families about the importance of behavioral health
• Maintain accurate and up-to-date patient records, documenting pertinent medical information.
• Collaborate with other healthcare professionals, including physicians, nurses, and support staff, to coordinate patient care and ensure continuity of care.
• Adhere to all relevant laws, regulations, and healthcare standards related to vaccination administration and patient care.
• Keep prompt communication with office staff (confirming appointments and sending in completed forms)
• Provide admission forms and progress notes within 24hours of initial patient visits
• Communicate professionally with patient and/or patient family before and throughout patient visits.

Job requirements:

• Must possess an active, unrestricted NY License
• Reliable Transportation/willing to travel
• Spanish speaking preferred.

This is a Part-time/per diem position, 1099 contract. Schedules are flexible around your availability. Travel mileage and/or metro cards are reimbursed.
