Mental Health Association of San Mateo County is Hiring a Community Case Manager Near UNKNOWN, CA

Community Case Manager

Definition of Position: Provides case management and support services for formerly homeless individuals with psychiatric disabilities and/or substance use disorders who reside in independent or supportive housing settings in San Mateo County.

Specific Job Duties: Under the direct supervision of the Case Management Services Coordinator and in consultation with the Clinical Services Manager the Case Manager is responsible for the following:

  • Maintaining regular contact with all clients-at least three contacts per month with each client and at least one of the contacts being in person.
  • Becoming familiar with other agencies and service providers in the county and developing good working relationships with these resources in order to provide clients with the support and assistance they need.
  • Ongoing assessment of clients’ independent living skills, utilizing occupational therapy services, as needed, to assist clients in developing the skills they need to maintain independent living.
  • Creating and facilitating social and recreational activities for the community, as well as educational and skill building workshops.
  • Maintaining frequent contact with landlords and, as needed, with the Housing Authority of San Mateo County to assure that client is complying with the rules and regulations of their lease and housing voucher.
  • Serving as the liaison between client and landlord as needed to avoid conflict with other tenants, assure payment of rent is up to date, and to resolve conflicts or answer questions as needed and appropriate.
  • Providing ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
  • Documenting all contacts with clients as required and assuring that documentation is filed in a timely manner.
  • Assuring that other clinical documentation including annual Care Plan and Annual Assessments completed in a timely manner.
  • Frequent contact with supervisor to inform supervisor of daily schedule and general status of caseload.
  • Regular participation in staff meetings and clinical consultations.
  • Other tasks assigned as needed.

Work Hours: Regular Work Hours will be 10:00 a.m. – 6:30 p.m. Tuesday through Saturday.

Qualifications:

  • B.A. in Psychology or a related field.
  • Prior experience working with people with psychiatric disabilities.
  • Ability to be flexible.
  • Ability to work with a diverse population.
  • Good computer and documentation skills.
  • Willingness to submit to Live Scan screening and ability to pass criminal background check by the Dept. of Justice.

· The job involves driving, so we require that you have a car, a good driving record, and proof of insurance.

Job Type: Full-time

Pay: $27.00 - $30.00 per hour

Expected hours: 40 per week
