Medical Temporaries, Inc. is Hiring a Pre-Release Case Manager Near Richmond, VA

Medical Temporaries, Inc. is currently seeking an experienced ***Pre-Release Case Manager***.
  • Minimum of 1 (one) year of experience required working in substance use treatment or a similar field and prior work with department of corrections with justice-involved adults or other vulnerable populations.
This is a FULL TIME/Direct Hire position requiring availability between the hours of 8am to 5pm (must be flexible with hours due to nature of position), Monday through Friday (may require availability on weekends, evenings, holidays or outside of core hours), main office located in Richmond, VA 23230. Sites covered Chesapeake, VA (23322), State Farm, VA (23160), Victoria, VA (23974), Jarratt, VA (23870).
Responsibilities :
A Day in the life of a Pre-Release Case Manager :
As a Pre-Release Case Manager you will be responsible for in supporting participants progress through the pre-release phase of re-entry. The Pre-Release Case Manager serves as a trusted advisor and accountability partner for participants assigned to their caseload who also monitors the progress and helps participants navigate through obstacles. Pre-Release Case Managers facilitate individual sessions with participants and serve as the designated leader for participant-facing team members responsible for delivering the integrated services according to the model. This position requires the Pre-Release Case Manager to be direct with the participants and must have the ability to tell participants what they NEED to hear, not what they WANT to hear. Qualified candidates must also be nurturing, have the ability to be warm, engaging, and kind, while also being assertive. Empathy will also assist the Pre-Release Case Manager in being successful. Candidate must have the ability to relate to participants' experience through shared or similar experiences.
  • Develops a comprehensive understanding of every participant on caseload
  • Conducts individual session with participant
  • Holds a session with each active participant in pre-release.
  • Documents all sessions in CAPS after completion and prior to being moved to another facility or within 4 hours after the session.
  • Participant documentation required: Attendance, Arrival and Departure Times, Engagement Levels, Attitude, Homework, and SIRP Notes.
  • Works collaboratively with Enrollment and Pre-Release Administrator, Pre-Release Career Coach, and Pre-Release Therapist.
  • Attends Community and Life Skills team meetings every two weeks and All Team Meetings.
  • Meets with supervisor once a month to provide updates and review progress toward organizational goals.
  • Attend all mandatory training sessions throughout the year at Headquarters located in St. Louis.
  • Maintains positive and professional relationships with participants, team members, volunteers, community partners and adheres to policies and procedures.
  • Adheres to the best practices established for the Pre-Release Case Manager:
  • Uses desk phone application for all phone communications with participants.
  • Ensure only previously approved materials are transferred to participants in pre-release. Distributing unapproved materials may put the program in jeopardy.
  • Escalates any instances of missed all outs for each session to prison staff and program supervisor.
  • Refers education and therapy issues to the pre-Release Career Coach or Pre-Release Therapist to help ensure participants receive a consistent level of care and that team member workload is equally distributed.
  • Provides services only to participants who are active or in good standing.
  • Involves Enrollment and Pre-Release Administrator in there is a need for any direct communications with participants' family members or trusted contacts.
  • Learns applicable technology and has a computer or mobile device available during business hours.
  • Deliver only approved CLS curriculum.
  • Review and understand the importance of the master schedule to team effectiveness and efficiency and in preparation of your next day's session.
  • Communicates with team members by documenting all case notes through the CAPS participant system, to document participant progress and develop strategies, while protecting sensitive information.
  • Documents participant-related activity in CAPS as quickly as possible after scheduled sessions are completed to help ensure accuracy of information.
  • Works to ensure the participant schedules are accurate, as displayed in CAPS, and that future activities are always scheduled; understands the importance of the master schedule to team effectiveness and efficiency.
  • Retains participant and team member trust by always meeting commitments.
  • Your work consistently reflects the organizational values of the program: courageous, transformative, compassionate, inclusive, accountable, collaborative, intentional and always act with integrity.
Working Conditions:
This position works in a correctional facility and the Pre-Release Case Manager may have to travel between correctional facilities and the organization's home office. A reliable automobile (company is working on a fleet of leased cars to provide...in progress), valid driver's license and vehicle insurance are required for this job.
Requirements for the :
  • Required: Bachelor's Degree in social work, counseling, psychology, or related counseling degree
  • Required: Valid Virginia Driver's License with an acceptable driving record that is confirmed through the Department of Motor Vehicles. Must provide documentation of current/active car insurance.
  • Required: Minimum of one year of experience working in substance use treatment or similar area
  • Required: Prior experience working with justice-involved adults or other vulnerable populations
  • Required: Ability to pass Background Check and Drug Screen
  • Required: Must be able to obtain DOC security clearance
  • Required: Excellent written and verbal communication skills and boundary setting skills
  • Required: Knowledge of services available to the participant to link them to assistance with housing, employment, and clothing/basic needs.
  • Required: Ability to travel within state and attend interviews, training, and other functions outside of the State of Virginia that will be funded by the organization. FLEXIBILITY IN SCHEDULE AND TRAVEL A MUST!!!
Preferred Qualifications for the Pre-Release Case Manager :
  • Preferred: Substance Use Credentialing (i.e. CSAC - Certified Substance Abuse Certificate through the Virginia Department of Health Professions or National Substance Use Certification)
  • Preferred: Personal experience or lived this experience.
  • Preferred: Crisis Prevention Intervention (CPI training)
Benefits:
  • $47K-52K base salary (depending on experience)
  • Target Bonus 15% but can reach 25%, 50%, 100% or more (two times annually for meeting goals)
  • Stipend (10% of salary for hours worked directly in correctional facility)
  • Leased Vehicle (with gas card provided) - this benefit is in progress
