Job Title: Direct Care Staff/Med Aide/Caregiver

Reports To: Director of Nursing Services, Nursing Services Manager

Job Summary: Administers or assists residents to self-administer medications and treatments; measures vital signs and weights; and preforms other health services tasks.

Communication with physicians, implements physician orders, reports and documents pertinent information. Adheres to home health, hospice, and emergency protocols. Acts as facility supervisor when management staff not present. Treat each resident with respect and dignity, recognizing individual needs and encouraging independence. Foster a homelike atmosphere throughout the residence.

Essential Functions and Responsibilities:

  • Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents.
  • Administer or assists residents to self-administer medications and treatments.
  • Measure vital signs and weights.
  • Collect, store and assures transport of certain specimens for laboratory studies.
  • Adheres to all medication room systems per policy
  • Implements physician orders
  • Documents pertinent information in residents charts and elsewhere per policy (ie..incident reports)
  • Communicates with physicians via fax, visit form and telephone.
  • Communicates with Director of Health Services according to specified criteria.
  • Adheres to home health and hospice protocols.
  • Assist residents with ADL’s
  • Adheres to emergency protocols, including performing CPR and administering first aid as necessary.
  • Acts as facility supervisor when management staff not present.
  • Attend education and training classes including as necessary to fulfill state or provincial regulations.
  • Reads staff communication records and maintains familiarity with resident records as required.
  • Provides services as indicated on resident treatment and service plans.
  • Follows proper procedures in emergency situations, and obtains additional assistance if necessary.
  • Demonstrates knowledge of and follows infection control procedures.
  • Observes problems, concerns, issues, observed with resident apartments, the building and the grounds and communicates them appropriately.
  • Observes changes in resident's status, needs, or preferences, promptly reporting all changes to Director of Nursing Services.
  • Observes all work, safety, and administrative rules.
  • Adheres to all established policies and procedures.
  • Records resident information as required.
  • Avoid loss, breakage, and waste of supplies and equipment.
  • Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security, work, safety and other emergency procedures.
  • Contributes to the development of a strong, cohesive team in department.
  • Trains other staff as directed by Director of Nursing Services.
  • Performs other duties as assigned by the Director of Nursing Services.

Employment Requirements:

  • Ability and willingness to perform all position responsibilities adequately.

  • Maintains acceptable attendance record and follows work schedule.
  • Organizes and utilizes time appropriately toward accomplishing assigned tasks.
  • Displays cleanliness, good grooming, personal appearance, and follows dress requirements at all times.
  • Ability and willingness to learn new skills.
  • Interacts in a professional manner with residents, general public, and co-workers. Displays willingness to assist co-workers when situation requires.
  • Ability to occasionally lift heavy objects (10 to 25 pounds).
  • Obtains First Aid Certification.
  • Ability to solve practical problems and deals with different situations as well as interpret a variety of instructions furnished in written, oral, diagram, or schedule formats.

Qualifications:

  • Working knowledge of equipment, supplies, and chemicals used in nursing operations.

Education and/or Experience:

  • Must have high school diploma or general education degree; prefer a Certified Nurses Aide.
  • Must have a minimum of 1 year related experience and/or training; prefer 2 years experience working with geriatric residents.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

Language Skills:

  • Ability to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to effectively communicate in English and understand and follow written and oral instructions.

Reasoning Ability:

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations:

  • First Aid
