Case Manager Reception and PlacementPOSITION SUMMARY: The Reception and Placement (R&P) Case Manager provides directservices to refugees to help them resettle and integrate into their new community. This roleinvolves providing resources and support to help refugees achieve self-sufficiency.ESSENTIAL JOB FUNCTIONS:● Maintain case management for refugee clients in accordance with state and federalfunded contracts● Develop strengths-based service plans with clients to identify barriers to self-sufficiencyand outline the necessary steps to overcome these barriers● Provide direct services to assist clients in problem solving● Connect clients with community networks and support systems to ensure their long-termintegration into the community (this may include working closely with volunteers who aresupporting clients)● Provide transportation for clients, including scheduling medical transportation orensuring clients understands and has access to public transportation, to attend requiredappointments and ensure that language access is provided at these appointments● Ensure that clients are being served in a linguistically and culturally appropriate mannerand provide education and advocacy when necessary● Keep in regular contact with active cases per contractual requirements, including home-visits, office meetings, community orientation, and phone calls● Maintain organized case files, complete accurate progress notes for all servicesprovided, submit necessary reports in a timely manner, and keep shared tracking formsup to date● Coordinate needed services through referrals to refugee service providers including theHealth Department, Employment Services, Social Security, DSHS and ESL providers● Maintain compliance with all relevant program/department requirements, including andnot limited to adhering to all record-keeping and reporting requirements.● Model and encourage direct, professional communication among staff members andfacilitate development of a positive staff support network.● Demonstrate awareness and sensitivity in the delivery of client services as it relates tothe cultural and socioeconomic characteristics of the populations served.● Other duties as assigned.QUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential dutysatisfactorily. The requirements listed below are representative of the knowledge, skill, and/orability required. Education and/or Experience: Two years of related case management work experience Credentials: n/aGeneral Knowledge, Skills and Abilities: Ability to work well under pressure and workeffectively in a multicultural setting, demonstrating excellent organizational skills andinterpersonal skills. General experience with Google Suite and Microsoft Office.Transportation: This position requires reliable transportation for business purposes, such asbetween work locations and community settings. This position is responsible for driving clientsin a personal and/or agency rental.Language Skills: Excellent communication skills with the ability to read, write, and speak inEnglish.Clearance: Standard LCSNW criminal clearance, including national sex offender. Motor vehiclerecords, valid driver’s license and auto insurance.The Perks: We offer a full benefits package with options for medical, dental, pharmaceutical and vision coverage; employer-paid short and long-term disability, as well as life insurance.
