We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our team. Position Overview:
As an RN Case Manager at Luna Del Valle Hospice, you will play a crucial role in coordinating and providing comprehensive care to our patients and their families. We are looking for candidates who are licensed Registered Nurses with experience in end-of-life care and a compassionate and empathetic approach to patient care.

Responsibilities:

• Conduct initial and ongoing assessments of patients' physical, emotional, and psychosocial needs.

• Develop and implement individualized care plans in collaboration with the interdisciplinary team.

• Provide skilled nursing care, symptom management, and medication management to patients.

• Coordinate care services, referrals, and resources to meet the needs of patients and families.

• Offer education and support to patients and families on end-of-life care and advance care planning.

Qualifications:

• Current licensure as a Registered Nurse (RN) in New Mexico.

• Experience in end-of-life care or hospice settings.

• Strong assessment and critical thinking skills.

• Excellent communication and interpersonal skills.

• Compassionate and empathetic approach to patient care.

• Ability to work effectively as part of a multidisciplinary team.
