We are currently seeking a Per Diem Hospice RN Admission Nurse to join our dedicated team. As the first point of contact for our patients, you will play a vital role in providing support and guidance during the initial stages of their hospice journey.

Job Summary:
The Hospice RN Admission Nurse is a crucial member of our hospice care team, responsible for conducting thorough assessments of patients referred to our services. With a focus on dignity and respect, you will develop personalized care plans in collaboration with the interdisciplinary team. Additionally, you will serve as a primary source of education and support for patients and families, ensuring a seamless transition into hospice care.

Responsibilities:

• Conduct initial assessments of patients referred to hospice services.
• Evaluate the medical condition, symptoms, and overall needs of patients.
• Collaborate with the interdisciplinary team to develop personalized care plans.
• Provide education and support to patients and families about the hospice philosophy and available services.
• Coordinate the transition of patients into hospice care, ensuring accurate documentation.
• Offer emotional support to patients and families throughout the hospice journey.
• Advocate for patients' needs and preferences within the hospice care team.
• Communicate effectively with healthcare providers and caregivers.
• Participate in ongoing education and training.
• Maintain accurate documentation of patient assessments and interactions.

Qualifications:

• Registered Nurse (RN) license in the state of practice.
• Minimum of two years of nursing experience, preferably in hospice, home health, or palliative care.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work effectively as part of a multidisciplinary team.
• Proficiency in electronic medical records (EMR) systems.
• Willingness to participate in continuing education and training.

Pay: $73,589.27 - $88,623.64 per year
